- Page 2 of 2 Request for Quote (RFQ) #: 36C25920Q0211 Axumin for PET/CT Imaging of Prostate Tumors This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation. **Questions shall be submitted in writing to Matt Zorch at paul.zorch@va.gov no later than 4:00 p.m. EST, March 13, 2020 ** Quotes are to be provided to Matt Zorch at paul.zorch@va.gov no later than 4:00pm EST, March 20, 2020. Quotes shall not be hand carried. Faxed quotes shall not be accepted. Emailed quotes are the only method of submitted quotes (4MB email limit). The Government reserves the right to make award solely on initial Quotes received. Offerors bear the burden of ensuring that all portions of the offer (and any authorized amendments) reach the designated office before the deadline specified in the solicitation. This RFQ shall be completed in its entirety, and signed and dated, failure shall not be considered for award. Requirement Description: Axumin for Positron Emission Tomography (PET) and Computerized Tomography Imaging of Prostate Tumors for the Oklahoma City VA Medical Center located at: 921 NE 13th Street Oklahoma City, OK 37104. This is a Request for Quote (RFQ) and the solicitation number is 36C25920Q0211. The government anticipates awarding an Indefinite-Delivery Indefinite-Quantity (IDIQ) contract with firm-fixed unit pricing resulting from this solicitation. The anticipated minimum and maximum amounts for the IDIQ contract are as follows: Minimum: $4,531.00 Maximum: $1,812,400.00 This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-04 effective January 15, 2020.
