- Amendment 0001 for the Cost Per Test Integrated System Chemistry Analyzer requirement at Langley AFB, VA is amended as follows: Correct purchase�description from ""units shall not be refurbished or remanufactured"" and change to ""The equipment shall have like new appearance, no frayed cords or wires, and all components and exterior compartments shall be fully operational. The equipment shall not have more than one prior installation and may have only had three (3) preventative maintenances performed on the units.� Awardee must show proof of performance maintenance history. The equipment cannot be older than 36 months from manufactured date. The unit(s) shall be replaced at life expectancy or if either unit fails unexpectedly with no feasible way to repair. Delivery of any replacement units will be staggered for QA processing. Must include a warranty for what is standard for newly produced or unused Chemistry Analyzers.""� Combined Synopsis/Solicitation � Solicitation Number: FM48002033465 � Purchase Description: Two Cost Per Test (CPT) Integrated System Chemistry Analyzers to be purchased by the 633d Medical Group, Pathology Flight, Joint-Base Langley-Eustis (JBLE), located on Langley Air Force Base (LAFB) in Hampton, VA.� Requested period of performance is 1 April 2020 through 31 March 2021 with 4 option periods ending 31 March 2025. Each unit shall include test equipment, associated power ware, and all validation supplies and regents.� Required support includes equipment repairs, labor, travel, and materials necessary to keep both units operational to manufacturer's specifications.� All equipment must be of the most modern technology available within the commercial industry.� Due to facility constraints, waste shall be self-contained without a need for plumbing or drains (dry). The equipment shall have like new appearance, no frayed cords or wires, and all components and exterior compartments shall be fully operational.� The equipment shall not have more than one prior installation and may have only had three (3) preventative maintenances performed on the units.� Awardee must show proof of performance maintence history.� The equipment cannot be older than 36 months from manufactured date.� The unit(s) shall be replaced at life expectancy or if either unit fails unexpectedly with no feasible way to repair.� Delivery of any replacement units will be staggered for QA processing.� Must include a warranty for what is standard for newly produced or unused Chemistry Analyzers.� The first unit shall be delivered within 15 calendar days of award.� The second analyzer is to be delivered 45 days after installation of the first analyzer. This is a Combined Synopsis/Solicitation Notice for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation/notice; responses/quotes are being requested, further written solicitation will not be issued. This solicitation will be posted to the Federal Business Opportunities (FBO) website as full and open competition. Upon receipt of all offerors� timely responses, the technicial expert within the Pathology flight will determine if all requirements are met in accordance with the attached PWS dated 28 February 2020.� Once the review process in completed, award will be made to the Lowest Priced Technically Acceptable (LPTA) offer received. � The 633d Contracting Squadron is issuing this combined synopsis/solicitation, FM48002033465, as a Request for Quote using FAR Part 12, Acquisition of Commercial Item, & FAR Part 13, Simplified Acquisition Procedures.
