- Page 4 of 4 NOTICE OF INTENT TO SOLE SOURCE Toshiba/Canon Model Aquilion One Vision CT Scanner Preventative Maintenance & Repairs Veterans Health Affairs, Network Contracting Office 5, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide maintenance & repairs on the Toshiba/Canon Model Aquilion One Vision CT Scanner as described below that will not interrupt the continuum of care. This requirement is defined primarily by existing hardware and software already in use at the Martinsburg VA Medical Center, 510, Butler Avenue, Martinsburg, WV 25405. The government is contemplating a Base +4 option periods Firm Fixed Price contract to satisfy this requirement and meet the needs of the patient population at the Martinsburg VA Medical Center. BACKGROUND The Toshiba/Canon Model Aquilion One Vision CT Scanner is a CT Scanner located in the Radiology Department of our VA Medical Center. This unit consists of hardware and proprietary software. SCOPE This service will be for the Toshiba/Canon Model Aquilion One Vision CT Scanner. The contractor will provide: Routine Preventative Maintenance to the equipment, On-Site Support field service, Consumable Parts, Non-Consumable Parts, Critical Updates to the Application Software, Functionality Upgrades to the Application Software, Interface Modification, Interface Conversion, FDB Updates, and On-Site Training. DELIVERABLES Turn-around time 2 business days. PLACE OF PERFORMANCE Martinsburg VA Medical Center 510 Butler Avenue Martinsburg, WV 25801 TRAVEL N/A CLASSIFICATION/SECURITY ePHI (Electronic Protected Health Information) is involved. Business Associate Agreement between the contractor and the Department of Veterans Affairs is required, in accordance with the Health Insurance Portability and Accountability Act (HIPAA), the Health Information Technology for Economic and Clinical Health Act (HITECH), and the HIPAA Privacy, Security, Breach Notification, and Enforcement Rules ( HIPAA Rules ), 45 C.F.R. Parts 160 and 164, for the Use and Disclosure of Protected Health Information (PHI) GOVERNMENT FURNISHED PROPERTY None.
