Description
This request for information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This notice is published to conduct market research to determine if there is a sufficient number of certified Service-Disabled Veteran Owned Small Businesses (SDVOSB) and/or Veteran Owned Small Businesses (VOSB), capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. It is requested that responses include a rough estimate of pricing for completion of the requirements identified below. Responses are due by April 17, 2020 at 1:00pm MST It is anticipated that a solicitation may be issued in Apr 2020. If issued, award of a subsequent contract is contemplated by May 2020. Responses should be submitted to nakia.vance2@va.gov NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. 1. Background and Purpose: This requirement is for a multi-year Preventative Maintenance and Calibration on Sotax/Pharmatron AT4 and ST50 Tablet Testers in use by the NMVAHCS CSP CRPCC located at 2401 Centre Ave SE, Albuquerque, New Mexico 87106.The VA Cooperative Studies Program in Albuquerque, New Mexico requires a service technician to travel to our facility and perform calibration and preventative maintenance on the Sotax/Pharmatron AT4 and ST50 tablet and capsule test equipment on an annual basis. Both pieces of test equipment are used to test tablets and capsules manufactured by the New Mexico Veterans Affairs Health Care System (NMVAHCS) Cooperative Studies Program Clinical Research Pharmacy Coordinating Center (CSP CRPCC) to verify they meet product specifications. These tablets and capsules are consumed by veteran participants in research projects conducted by NMVAHCS CSP CRPCC 2.General Information: Contract service shall ensure that the Test Equipment functions in conformance with the latest published edition of the applicable standards, such as: NFPA-99,UL, OSHA, VA, CDRH, etc. and the Manufacturer's Performance standards/ specifications as used when the equipment was originally procured and that any upgrades/updates will meet the stated standards/specifications. Equipment is listed below: 3. Services Required: Services will be performed on Sotax/Pharmatron AT4 serial #041200481 and Sotax/Pharmatron ST50 serial number 301.0579 4. Security and HIPPA Compliance Considerations: In reference to VHA Handbook 6500.6 Appendix A, Block 6, the equipment (or service) included in this contract does not involve connection of IT devices to a VA network. Therefore, C&A, SAP, & the following from App C apply In reference to VHA Handbook 6500.6 Appendix A, Block 7, the equipment (or service) does not involve storing, generating, transmitting, or exchanging VA sensitive information.� Therefore, the following from App C apply. (Appendix B) The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. During contract performance, contractor may require access to VA facility, equipment, information systems, or sensitive data; to provide services, install, train, maintain or repair equipment. VA Course No. VA10176 is privacy training required for VA contractors requiring access to VA facilities for more than 2 consecutive days or any access to VA information systems.� Training will be completed prior to being granted access to facilities, equipment, or to the data therein.� Training is available on VA TMS website at: https://www.tms.va.gov/SecureAuth35/.� Contractor will provide evidence of training to the COR, if requested. If contracted staff have access to protected health information (PHI) or access to VA systems (e.g., CPRS, JLV, CAPRI, VHIE or VistAWeb) they are also required to take the Privacy and HIPAA Focused training (VA10203). 5.
