This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05 / 03-30-2020. The Network Contracting Office 7 is seeking to courier services for the Atlanta VA Medical Center and surround medical clinics. All pick-ups and deliveries will be allotted a +/- 15min grace period. The first pick-up of the day at the CBOCs is no later than 11:00 AM -11:30 AM. Monday through Friday. The items picked up at 11:00 AM - 11:30 AM shall be delivered to the Atlanta VAMC between 1:15 PM 1:45 PM. The second pick-up of the day at the CBOCs will be at 3:30 PM - 4:00 PM daily (with the following exceptions: Blairsville, Atlanta VA Clinic, Rome, Ft. McPherson and Trinka Davis Veteran Village). The items picked up at 3:30 PM - 4:00 PM in the CBOC shall be delivered to the Atlanta V.A. Medical Center no later than 7:00 PM. Exceptions to these delivery times are outlined in 1.1.3 through 1.1.7. Trinka Davis Veteran Village: Additional deliveries outside of normal business hours are required at the Trinka Davis Veteran Village on a case by case basis. Courier services must have representatives available to meet the clinic s needs 24 hrs/day, 7 days/week. Upon pick up of package from clinic, Vendor will have (2) two-hours to deliver to medical center. The Trinka Davis Veteran Village is approximately 50 miles one way from the Atlanta VAMC. Rome Outreach Clinic: The Courier Service shall transport mail, laboratory specimens, X-Ray films, medicine/prescriptions, assorted packages and other items on a twice daily basis from the Rome Outreach Clinic to the Atlanta VA Medical Center in Decatur, GA. The first pick-up of the day at the Rome Clinic is 11:15-12:00 noon Monday-Friday. The second pick-up of the day at the Rome Clinic will be at 5:30-5:45 PM Monday-Friday. Return trips from the Rome Outreach Clinic to the Atlanta VA Medical Center shall be completed within 3 hours. The Rome Outreach Clinic is approximately 72 miles one way from the Atlanta VAMC. Blairsville CBOC: The Courier Service shall transport mail, laboratory specimens, X-Ray films, medicine/prescriptions, assorted packages and other items. The first pick-up of the day at the V.A. Medical Center is 12:15-12:45 PM Monday through Friday. The items picked up at 12:15-12:45 PM shall be delivered to the Blairsville CBOC between 3:15 4:00 PM. The second pick-up of the day at the Blairsville CBOC will be at 3:15 4:00 PM daily. The items picked up at 3:15 4:00 PM in the CBOC shall be delivered to the Atlanta V.A. Medical Center no later than 7:00 PM. The Blairsville CBOC is approximately 107 miles one way from the Atlanta VAMC. Atlanta VA Clinic: The Courier Service shall transport mail, laboratory specimens, X-Ray films, medicine/prescriptions, assorted packages and other items on a twice daily basis from the Atlanta VA Clinic to the Atlanta VA Medical Center in Decatur, GA. The first pick-up of the day at the Atlanta Clinic is 11:15-12:00 noon Monday through Friday. The second pick-up of the day at the Atlanta VA Clinic will be at 5:30-5:45 PM Monday through Friday. Return trips from the Atlanta VA Clinic to the Atlanta VA Medical Center shall be completed within 1 hour. The Atlanta VA Clinic is approximately 3 miles one way from the Atlanta VAMC. Ft. McPherson: Additional deliveries outside of normal business hours are required at the Ft. McPherson CBOC on Saturdays. Courier services must have representatives available to meet the clinic s needs. Upon pick up of package from clinic, Vendor will have (2) two-hours to deliver to medical center. Additional clinics may be added in the future for Saturday puck ups. The Ft. McPherson CBOC is approximately 15 miles one way from the Atlanta VAMC Courier will be responsible for retrieving items from VA Primary Care Service Line to deliver to the outpatient clinics upon leaving the medical center after their first delivery. Items are to be delivered to the outpatient clinics upon arrival for second courier pick up. The Courier will not have a need to use any VA Information System to log into the network at any time. The contractor shall deliver items to the appropriate Atlanta V.A. Medical Center departments. All items will be placed in containers that will be sealed by VA employees prior to pick-up. Prescriptions/medicine shall picked-up in sealed envelopes (sealed by VA employees prior to pick-up) and delivered to the first-floor pharmacy window. The Courier shall sign the Lab Tracking Form at the time of pick-up at the Clinics. Laboratory specimens and Lab Tracking Form shall be placed in a locked/sealed transport bag prior to courier pick-up. The transport bag will be delivered to the laboratory receiving area on the second floor of Ambulatory Care, Building B. The laboratory receiving area will maintain the key for the locked/sealed transport bag. Courier is responsible for obtaining signature of lab personnel receiving specimens and ensuring that each bag number corresponds to the bag number on the Lab Tracking Form. The Courier will return the completed Lab Tracking form to the Primary Care administrative section located in Room 6C138A at the Medical Center. X-ray films shall be delivered to a designated area in the Radiology Service as needed. The Courier shall note the time of the delivery to Radiology Service in the CBOC logbook. Mail and other assorted packages shall be delivered in a locked/sealed transport bag to the designated mailboxes on the sixth floor of the Atlanta VA Medical Center located on 1670 Clairmont Road. Decatur, GA 30033. The contractor is prohibited from having a key for the bag. The contractor is prohibited from transported unlocked/unsealed transport bags. All keys will be maintained by VA staff. Mail and packages for the Blairsville CBOC shall be picked up (from designated area on the sixth floor) at the Atlanta V.A. Medical Center between 12:15 - 12:45 PM daily and delivered to the CBOCs no later than 3:15 - 3:45 PM daily. Delivery to the Atlanta VA Medical Center of mail, specimens and x-rays is expected to be accomplished at the daily courier run. No item shall be held overnight. If the courier should find a specimen or any other VA property after making a delivery, those items must be returned to the Atlanta VA Medical Center the same day. The Blairsville CBOC will call the Courier Service by 10:30 AM to stop the courier run if necessary. The Atlanta VA Medical Center reserves the right to adjust the pickup hours as needed. The COR will notify the contractor within 2 days if there is a change in the pickup or delivery schedule. Mail and packages for the remaining CBOCs shall be picked up (from designated area on the sixth floor) at the Atlanta V.A. Medical Center between 1:15 1:45 PM daily and delivered to the CBOCs no later than 3:45 - 4:15 daily. Delivery to the Atlanta VA Medical Center of mail, specimens and x-rays is expected to be accomplished at the daily courier run. No item shall be held overnight. If the courier should find a specimen or any other VA property after making a delivery, those items must be returned to the Atlanta VA Medical Center the same day. Each CBOC will call the Courier Service by 9:00 AM to stop the afternoon courier run if necessary. The Atlanta VA Medical Center reserves the right to adjust the pickup hours as needed. The COR will notify the contractor within 2 days if there is a change in the pickup or delivery schedule. Transport Bags: All transport bags will be locked/sealed by VA staff prior to the contractor accepting the transport bag. The contractor is prohibited from having a key (or any other measure to break the seal) at any time. The VA staffs will the key for the locked/sealed transport bags. Transport bags will be provided by the VA. Emergency Courier Service: The contractor shall provide emergency pick-up and delivery of special laboratory specimens, mail, x-ray films and other assorted packages should the need arise (Monday, Tuesday, Wednesday, Thursday and Friday between the hours of 8:00 AM to 6:00 PM). Approximately two (2) hours notice will be given for the special service. Emergency runs will require same day turn around service. The COR or designee will approve the arrangements for the emergency pick-up and delivery. The contractor will be reimbursed for emergency runs based on the price in this contract. Contractor shall transport samples in such a manner as to ensure the integrity of the specimens. OSHA approved laboratory containers/coolers with refrigerant packs will be provided by the Contractor. All containers/coolers will be labeled with a biohazard label. Contractor shall be responsible for complying with all regulations that define appropriate transportation of specimens and shall supply, in writing, proof that all drivers have been adequately trained on an annual basis to the COR. The contractor's employees/couriers transporting test specimen must be trained in ""Universal Precautions"" in the handling of biohazard materials (specimens). Training must include utilizing clinical procedures in case of biohazard spills and accidents. The contractor must furnish a record of all required training to the Contracting Officer. Saturday Courier Service: The Contractor shall provide one Saturday pick-up from CBOC and delivery to Atlanta VA Medical Center of mail, laboratory specimens, X-Ray films, medicine/prescriptions, assorted packages and other items when scheduled two days in advance NLT 3:00 PM or as otherwise established by the contract. PERFORMANCE MEASUREMENT Outcome: The contractor will provide qualified personnel who will fulfill the requirement to provide courier services at a standard enabling the Atlanta VAMC to provide such services at a high level of quality. How Measured: The Contractor must maintain a record and log of daily courier pick-ups and deliveries indicating time and date of pick-up, delivery, number of specimens and/or items and signatures. A Lab Specimen transport bag tracking sheet will be signed by the courier and submitted to VA Lab located on the second floor of building B at Atlanta VAMC located on 1670 Clairmont Road, Atlanta, GA 30033. Transport bag tracking sheet will be time stamped upon arrival. Courier is required to return signed sheet to Primary Care Service Line located on the sixth floor of building C at Atlanta VAMC located on 1670 Clairmont Road, Atlanta, GA 30033. Lab specimen transport bag tracking sheet will include the following: Name of Clinic Date Bag # Signature of Courier picking up bag Time bag picked up by Courier VA lab representative who received transport bag Section for lab time stamp Period of Performance: This contract is effective for one (1) year from date of award plus four (4) one (1) year renewal periods that may be exercised by the VA. The contract is subject to the availability of VA funds. The contractor shall perform no services after the end of the base year period of performance until the contracting officer authorizes such services in writing. Communications with staff should be open and immediate, to resolve professional issues. Patient satisfaction is paramount procedural issues should be immediately resolved with the designated Contracting Officer s Representative (COR). Quality Control: The contractor shall develop and maintain a quality control program that ensures courier services are performed in accordance with these specifications. The contractor shall develop and implement procedures to track, identify, prevent, remedy and ensure non-recurrence of defective services. The contractor shall provide a method to accept and resolve customer complaints and notify the customer of the resolution. The contractor shall immediately notify the COR upon receipt of a customer complaint in order that joint validation may be accomplished. The contractor is responsible for quality control and specification compliance. Record of Pick-up and delivery: The Contractor must maintain a record and log of daily courier pick-ups and deliveries indicating time and date of pick-up, delivery, number of specimens and/or items and signatures. QUALIFICATIONS Contractor couriers/drivers: All contractor employees must maintain a valid driver's license for the type of vehicle operated as required by the States operating therein. Contractor personnel shall wear visible identification at all times while on the premises of any Federal property. It is the responsibility of the Contractor to park in the appropriate designated parking areas. A record of each driver as to character and physical capabilities for performing as a courier must be maintained and made available to the Department of Veterans Affairs Medical Center upon request for inspection. The contractor personnel performing services to the Government shall at all times conduct themselves in a professional manner, present a neat appearance and be easily recognized as a contract employee by having the company name, and employee name affixed to the clothing or uniform. The Contractor shall be responsible for furnishing all vehicles, personnel, equipment and supplies, vehicle fuel, uniforms, name tags and/or badges for performance of services under this contract. Employee identifications shall be worn in clear view above the waist. All contractor employees responsible for transporting specimens must have been trained and certified in accordance with OSHA (Occupational Safety and Health Organization) requirements. The Government reserves the right to restrict any contractor employee from performing services under this contract who do not meet the required qualifications, or who violate Federal regulations or are identified as a potential threat to the security, safety, health and/or operational mission of VA and its veteran population. The restriction of such contractor personnel shall not relieve the contractor from performing all the required services in accordance with all terms, conditions, and schedules contained herein. Contractor Vehicles: The vehicles used in the performance of this contract must be licensed and meet the minimum requirements by the States and cities operated therein. CONTRACTOR FURNISHED PROPERTY AND PERSONNEL SERVICES The contractor shall provide the following for all personnel furnishing services under this contract: Workers compensation Professional liability insurance Health examinations Income tax withholding Social security payment The parties agree that the contractor, its employees, agents, and subcontractors shall not be considered VA employees for any purpose and will be considered employees of the contractor. The responsibility for any leave, including any sick leave, professional development or vacation time, is the responsibility of the contractor. The government may evaluate the quality of professional and administrative services provided but retains no control over the professional aspects of services rendered. MONITORING Upon contract award, the Contracting Officer will appoint a Contracting Officer's Representative (COR) to assist with the contract monitoring requirement. The COR, VA Medical Center, Atlanta, GA shall be the VA official responsible for verifying contract compliance. After contract award, any incidents of contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. The COR will be responsible for verifying contract compliance. Any incidents of contractor non-compliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. Documentation of services performed shall be reviewed prior to certifying payment. The COR will perform periodic spot checks and document with the using service to ensure reports monitoring. VAMC will pay only for services actually performed and in strict accordance with the schedule of prices/cost. Contract monitoring and record keeping procedures shall be sufficient to ensure proper payment and allow audit verification that services were provided. CONTRACTOR FURNISHED PROPERTY Contractor Furnished Items and Services: The contractor shall furnish all necessary items and services required to perform this contract in accordance with the terms of this PWS and the contract except for those items or services specifically stated to be Government furnished. PAYMENT Each month, an itemized invoice which includes both the contract and purchase order number shall be submitted electronically mailed to the COR or Administrator of Primary Care, Atlanta V.A. Medical Center for the payment. The invoice for monthly services is to be received no later than close of business on the fifth of the month following the service. Payment to the contractor shall be within 30 days in arrears. The Atlanta VAMC will only pay for services actually performed and in strict accordance with the Schedule of prices/cost in the contract. DEFINITIONS Trip = One Way: The runs should be priced separately (per each trip). One run is considered to be (one way) from the Atlanta V. A. Medical Center to the CBOC/Outreach Clinic and vice versa. Parking: Parking at the CBOCs is available near the front door of each clinic. Parking at the Atlanta V.A. Medical Center is available in Emergency Parking at the Emergency Room Entrance to the medical center. Parking is free of charge at both locations. Specimen Transport: Laboratory specimens are to be transported in OSHA approved biohazard coolers with refrigerant packs (provided by the Atlanta VA Medical Center) to maintain appropriate temperature for laboratory specimens. Mail Transport: Mail is transported in locked security pouches provided by the Atlanta V.A. Medical Center. X-Ray Film Transport: X-ray films are transported in locked canvas pouches provided by the VA as needed. DATA SECURITY INFORMATION GATHERED OR CREATED IN PERFORMANCE OF SERVICES Information, including but not limited to veteran individually-identifiable information and personal healthcare information (PHI), gathered or created by the contractor in the performance of this contract is the exclusive property of VA and must be received, gathered, stored, backed up, maintained, used, disclosed and disposed of in accordance with the terms of this contract and applicable federal and VA information confidentiality and security laws, regulations and policies, including VA Directive and Handbook number 6500. The contractor shall immediately (on the date of discovery of the incident) report any incident of theft, loss or compromise of VA sensitive information to the Contracting Officer, the Contracting Officer s Technical Representative, and any other VA Official designated by the Contracting Officer. This report shall be in writing and contain the fullest explanation possible and all the details of the incident available to the contractor. VA must receive notice from the contractor on the date contractor discovers incident. Award shall be made to the SDVOSB who offers the BEST VALUE to the Government, considering price, past performance, and meeting the specifications outlined in the PWS.
