36C24920Q0263 NOTICE OF INTENT TO SOLE SOURCE AUDIOLOGY EQUIPMENT 2 | Page This procurement is being conducted in accordance with (IAW) 41 USC 3304(a)(1) and FAR 13.106-1(b), Soliciting from a Single Source. The NAICS code is 334510 with a size standard of 1250 employees. The Department of Veterans Affairs (VA) NCO-9 and Memphis VA Medical Center s [Facility] intends to issue a sole source firm-fixed priced purchase order to E3 Gordon Stowe for the procurement of the items in the Description of Requirement section below. DESCRIPTION OF REQUIREMENT: Small, single-walled sound booth for the Jackson, TN CBOC to complete hearing assessments that meet ANSI noise reduction standards and meet VA handbook minimum criteria. Exterior dimensions needed to fit in space are no greater than 4 4 wide x 4 4 deep x 7 9 ; interior dimensions needed would then be 3 8 wide x 3 8 deep x 6 6 high. Breakdown: C-11A LP2 1 Eckel Audiology C-11A - CL-Series 1 EA LP2 (Single-Wall Camlock Enhanced) Audiometric Rooms - 2 1/2"" (64 mm) Rise Isolated Floor SOUND ROOM 2 20""W X 60""H Window in Door 1 EA 8120006 3 Soundroom Kit for Maico MA41 1 EA 8069 4 Sound Room Installation - includes 1 EA pre-install consultation & site measurements all labor (non-union rate) materials such as caulk, fiberfill insulation, fasteners touch-up. Customer is responsible for disposal of all packaging materials. Ceiling height must be 2"" higher than the highest section of the booth. Booth will need to be 4 inches from the wall. All ceiling tiles must be removed by the facility (if necessary). Installation done during normal business days and hours. 8070 6 Audiometer M41 Installation - Includes sound room certification 1 EA Justification for brand and sole source to designated vendor: Eckel C11-A Low Profile is needed for the following reasons: Only booth company that uses true cam-lock wall and ceiling construction. Relocation may be required as the space is leased. This exact product can be easily deconstructed and moved without compromising the integrity of the acoustic panels. This company and product meets the very small space requirements needed in Jackson. Standard left-hinged door- required for the layout of the dual-purposed room in Jackson. Low profile step up needed; ramp will not fit in room. Ventilation system silencer ceiling mounted; no room for external ventilation system in the space in Jackson and this will prevent need for building modification to use leased space s air ducts. This booth was ordered based on measurements and orientation completed on-site in Jackson. Part of the order is for the Soundroom Kit for Maico MA41 audiometer this is the audiometer on-site in Jackson that will be used for this booth. 20 by 60 Window on door needed for veteran population in an enclosed space. E3 Gordon Stowe under the E3 Diagnostic Group is the only authorized dealer/installer for this equipment. Deployment (Installation & Integration) Shipping/Freight based on delivery to a dock to zip code 38305. If shipping/freight is more than $250.00, the Government chooses to use its government-contracted shipper/freight provider and requires completion of form VA134a. Delivery Time: 7-9 weeks after approval of drawings The total estimated single purchase cost for this product and attached services is expected to be below $15,000.00. Market Research Results: The leading sound booth company is Acoustic Systems. However, they do not offer true cam-lock wall construction and do not offer a single-walled booth smaller than 7 0 by 6 8 exterior without custom construction.  Therefore, the basic requirements for removal and size cannot be met. Another sound room company IAC Audiometrics offers only a pre-constructed welded booth of comparable size. No cam-lock option. No long window available for this booth. Small Maico booths are lacking the cam-lock construction and large door window needed for veteran population.  QuietTest booth lacks cam-lock and is too small for our patients. SCOPE OF WORK: The Facility seeks to procure this equipment, and have it completely installed before September 30, 2020. No options for additional quantities or extension of terms of service were considered.
