Submission Formats:� Vendors can choose to submit a quote in any format as long as the quote clearly provides all of the following information: Items quoted Including information detailing how the items quoted meet the specifications of the requirements description.� This includes but is not limited to the required sample menu. Quantities quoted Unit Prices Extended Prices Delivery information Total Price Vendor�s Company Name Vendor�s Current CAGE Code (SAM Registered) Vendor�s Point of Contact (Individual with authority to sign a contract for the vendor) and their contact information) Other Vendor Points of Contact (Sales; Billing, etc; if desired and different than the individual with signature authority) and contact information Past Performance reference information. See evaluation factors below. See attached requirements description for delivery requirements including dates, times, address, and requirements for accessing Camp Dodge, IA installation Attached is a list of the applicable clauses and provisions for this contract requirement.� The Government will evaluate quotes using a Best Value approach.� The following three (3) evaluation factors apply: Price:� Quotes will be evaluated based on the total price of all items requested.� The Government intends to award one contract for all the requested items. Technical:� Items and specifications (such as the sample) quoted, meet the agency�s needs.� Past Performance:� �Vendors must submit three (3) references with a point of contact, that individual's phone number, the contract number, and the dollar amount of the contract.� References should be within 3 years prior to the date of publication of this requirement. Award:� The Government intends to award a Firm Fixed Price contract.� Award will be made to a vendor who is deemed responsible and offers the best value for the items that meet the customer�s needs.� We intend to award a contract for this requirement no later than 1:00 PM on 2 July 2020.
