AMENDMENT 0004 a.� Amendment 0004 is hereby issued to extend due date, update combines specificatations, and update evaluation factors.� � � �1.� Block 13a of SF1442, changes due date from ""6/22/2020"" to read ""6/24/2020"". � � �2.� Update Combined Specifications dated 2019 05 03, pages 337-349 have been correct/made legible as attached document. � � �3.��Update VI.B(3) Lowest Price Technically Acceptable Evaluation Process: Factor 1 - Experience on Similar Projects Evaluation Criteria: This factor evaluates the extent of the Offeror�s past experience as a firm in providing similar construction and modernization services to determine the firm�s experience in completing the tasks detailed in the construction documents. Bidders must submit a minimum of three (3) completed projects that met the following minimum criteria: Project involved modernization of an elevator system of a comparably sized structure; Project was performed in an occupied and functioning office building; Project involved all of the following disciplines - architectural, civil, structural, HVAC, plumbing, electrical, fire alarm, elevator, and telecommunications; Project included requirements for noise control The total project construction cost at award of the construction contract(s) was not less than $2 million. Project was completed in the last 5 years. High Confidence- Project experience on similar projects was submitted with multiple similar projects, and a record of success that inspires high confidence in their ability to oversee this project. Adequate- Project experience on similar projects was submitted that show sufficient minimal experience. All three projects meet the minimum criteria. Questionable- Project experience on similar projects were submitted that raise questions or a lack of confidence in the firm OR the minimum criteria requirements were not submitted. Factor 2 - Past Performance on Current/Previous Projects There is no submission requirement for this factor. The evaluation team will review previous projects and contracts and if necessary CPARs. This factor will be rated using the following Confidence Based rating scheme: High Confidence: The offeror has record of success on past projects that inspires high confidence in their ability to complete this project. Adequate: The offeror has average past performance history. The offeror's may not necessarily have anything that raises significant concerns about their ability to complete this project but also falls short of inspiring high confidence. Questionable: The offeror's past performance gives reason to be concerned about their ability to successfully complete this project. Offeror's with no past performance history will be rated adequate. Factor 3 - Key Personnel The GSA Project Team has determined that this project will benefit significantly from an experienced full-time onsite Superintendent and therefore has decided to evaluate the experience of the offeror's proposed Superintendent as part of this RFQ. This factor will be evaluated by reviewing the submitted resume and evaluating how individual's experience, education, and qualifications relate to this specific scope of work. The proposed superintendent must show a minimum of five (5) years'�experience in a similar role. These factors will be rated using the following Confidence Based rating scheme: High Confidence- Resume details a very experienced Superintendent with multiple similar projects, and a record of success that inspires high confidence in their ability to oversee this project. Adequate- Resume shows sufficient minimal experience, that the Superintendent likely has enough experience to oversee this project but falls short of inspiring high confidence for one or more reasons. Questionable- Resume raises questions or a lack of confidence that the Superintendent has sufficient experience to successfully oversee this project for a number of discernible reasons. The Offeror must provide a resume for the following Key Personnel that includes the information identified below and must meet the minimum requirements stated here. On-Site Construction Superintendent Project Manager for Construction Quality Control Manager Project Scheduler All resumes must include the following information and explain how the proposed Key Personnel�s experience meets the minimum requirements. Each Resume cannot exceed two (2) pages: Name and title Project assignment Name of firm with which associated Years of experience with this firm (in the key personnel�s field of expertise/discipline) and years of experience with other firms (in the key personnel�s field of expertise/ discipline) Project Manager for Construction � must possess minimum of 5 years On-Site Construction Superintendent � must possess minimum of 5 years Quality Control Manager � must possess minimum of 5 years Project Scheduler � must possess a minimum of 2 years Education degree(s), year, and institution Other experience and qualifications relevant to same/similar work required under this contract List of projects in which the individual has worked to include Name of project(s) and project location(s) //////////////////////////////////////////////////LAST ITEM//////////////////////////////////////////////// AMENDMENT 0003 a.� Amendment 0003�is hereby issued to extend due date and to make correction to SF 1442 block 13d.� b.� Block 13a of SF 1442, is hereby changed from ""6/5/2020"" to read ""6/22/2020"". �� c.� Block 13d of SF 1442, add ""120"" to this block of the form.� �� d.
