SOW Modular Cabinetry for First Floor #607-20-3-081-0416 Page 7 of 7 RFI This is a request for information (RFI) regarding a procurement for Modular Cabinetry and Installation of Clinic desks in accordance with the Statement of Work and Salient Characteristics provided. If a contractor is able to provide these supplies and services, please response to this request by contacting Amy.Burger@va.gov by 5:00PM CST on June 26, 2020. No phone calls. The VA does not require an official bid proposal or specifications on that date. The Government only requires a signed letter and/or statement that the Contractor can provide the supplies and services in accordance with the Statement of Work. If the Government receives two or more responses, the Government will proceed with a request for quotes, at which time the Contractor will be required to submit a quote with drawings and specifications. Please note substitute materials are prohibited. STATEMENT OF WORK Modular Cabinetry and Installation for the first floor Clinic/Information desks Wm. S. Middleton Memorial VA Hospital 2500 Overlook Terrace Madison, WI 53705 TRANS: ____607-20-3-081-0146________Modular Cabinetry BACKGROUND: This modular cabinetry order addresses the need for all new clinic desks and front information desk on our first floor. This cabinetry needs to match the same style & finish that was installed in other areas to give a uniform look in our hospital, as well as the ease of maintenance. These are items specified to materially, functionally and aesthetically fit the needs for the modular desks. For this type of space, it is important to have the 3D thermofoil laminate that will provide antimicrobial & wipeable surfaces that will not delaminate or have edging. OBJECTIVES: Provide modular cabinets, walls, counters for our first-floor main information desk & clinic desks SCOPE: Finalize, design & component specifications, & installation drawings and timelines for review & approval by VA. The vendor will receive the components from the manufacturer and install all items which must meet or exceed the specified manufacturers per the following technical selection criteria. General descriptions of the seating are as follows: GENERAL SPECIFICATIONS: (MTR Code: MM1) All cabinetry & millwork to have 3D /Thermofoil, Grade 2 Orissa Cherry wood standard laminate, vertical grain. Hardware: 107 degree swing hinge with 2 hinges per door up to 36 , 3 hinges per door on extended height cabinets. Standard Bow Pulls, Finish: Brushed NIckel Drawers are std. with Tandembox premium series full extension drawer system dynamic load capacity of 75lbs and 100lbs. All file drawers are std. equipped with a 6mm aluminum file bar. Std. pulls are a bow pull #117.31.632 from Hafele and a Rod Pull #305128195 from Richelieu or equivalent. Shelf supports shall be flattened shelf support pins of 5-millimeter nickel. Grommets are 3-inch black plastic. Cabinet shelves are ¾ thick MDF and finished on all sides Cabinet backs are ¾ thick MDF laminated with white thermally fused melamine on the interior side for 3D laminate cabinetry The 3D laminate is heat formed around 5 surfaces to create a seamless, self- edge that won t chip, crack or delaminate. WALL FINISH SCHEDULE: PRODUCT SUMMARY- (Please see casework/wall panel shop drawings for exact specifications/measurements): A1049/ Travel Desk: 88 long desk, built between the existing walls 1.5 thick vertical walls with 1.5 thick solid surface top cap 1.25 thick snow white thermofoil Sit Stand surface, 24 d x 60 w (Electric Base by VA) 30 high ADA SS countertop Cabinet with doors & drawers Std Bow pull hardware 9 high toe kick, to provide the ease to replace this section if damaged from wheelchairs. D1199/ Information Desk Main desk is 153.2 with storage towers on each side. 1.5 thick vertical walls around the perimeter of desk Cabinet with doors & drawers 30 high ADA SS countertop Std Bow pull hardware 9 high toe kick, to provide the ease to replace this section if damaged from wheelchairs. Data/Electrical as noted on the panel shop drawings D1189 Walls /By Lake Clinic & Outpatient Pharmacy 7 -1-1/2 walls consisting of thermofoil panels on the bottom; (M8) SS top cap, and etched, tempered glass above 3 -4 half walls w/ glass above 9 toe kick reveal at base D1304 Bluff Clinic Desk 1.5 thick vertical walls with 1.5 thick solid surface top cap 1.25 thick snow white thermofoil Sit Stand surface, 24 d x 60 w (Electric Base by VA) Cabinet with doors & drawers Std Bow pull hardware 9 high toe kick, to provide the ease to replace this section if damaged from wheelchairs. Data/Electrical as noted on the panel shop drawings D1264 Cabin Clinic Desk 1.5 thick vertical walls with 1.5 thick solid surface top cap 1.25 thick snow white thermofoil Sit Stand surface, 24 d x 60 w (Electric Base by VA) Cabinet with doors & drawers Std Bow pull hardware 9 high toe kick, to provide the ease to replace this section if damaged from wheelchairs. Data/Electrical as noted on the panel shop drawings D1265 Walls /By Cabin Clinic & Infusion Clinic 5 h walls consisting of (3 -4-1/2 ) thermofoil panels on the bottom; (M8) SS top cap, and 1 -6 h (3/8 thick) clear tempered glass above 9 toe kick reveal at base to provide the ease to replace this section if damaged from wheelchairs. Destination charges to the Wm. S. Middleton Memorial Veterans Hospital, Madison, WI are included in the contract price. NOTE: Notwithstanding, the Contractor shall perform no additional work without the prior written authorization of the Contracting Officer. Any additional work, which may be performed by the Contractor without this approval, will be performed by the Contractor at no additional cost to the Government. No C&A or MOU/ISA is required. Sensitive information exposure as part of this contract involves applicable security controls within the facility as part of the VA Information Security Program. INSTALLATION: Install materials and systems in accordance with manufacturer published instructions and requirements. Install materials with uniform appearance and in proper relation with adjacent construction. Accurately set all cabinets plumb, square level, and permanently secure in position as indicated on the drawings. Install millwork to a tolerance of 1/8-inch or tighter in 96 inches for plumb and level. Install millwork without distortion so that doors and drawers fit openings properly and are accurately aligned. Adjust hardware to center doors and drawers in openings and to provide unencumbered operations. Exercise extreme care to avoid damaging the millwork finish during the handling and installation. Field fabrications and modifications of millwork required for completion of units shall be performed with extreme care to avoid damage to millwork finish. Scribe millwork and fillers as required for a tight fit. Wall cabinets shall be securely fastened to studs or blocking in the wall. Install countertops on base cabinets and other support systems with approved silicone sealant/adhesives. Secure backsplashes to tops with approves silicone sealant/adhesives. All cardboard and packing debris shall be removed and recycled by the installers. GUARANTEE/WARRANTY: The Vendor is an authorized dealer. The Manufacturer/Seller warrants the installation against defects of materials and workmanship under normal use and service for one year from the date of installation. Manufacturer/Seller will field-repair or replace any defective parts, including reasonable labor and shipping costs incident to repairing or replacing any such defective parts to the satisfaction of the Government. Manufacturer/Seller shall provide a copy of the written warranty upon delivery of the equipment. Product shall have a 10 year warranty on the construction of the millwork. SELECTION CRITERIA: Submit a proposal on company letterhead for the above outline s products and services. Material and installation/delivery charges are to be listed separately. Only EQUAL or BETTER items on GSA schedule will be considered. DELIVERABLE/INSTALLATION SCHEDULE: The contractor will provide safe and timely delivery and installation of all materials as outlined. Only product of EQUAL or BETTER items on GSA schedule will be considered. Delivery and installation are to occur no later than September 14, 2020. Installation should be completed between 7:00am 6:00pm Monday through Friday, except when approved in advance by the Contracting Officer. INSTALLATION PLACE/POLICY/PERIOD OF PERFORMANCE/PROCEDURES: Cabinetry is to be delivered and to the warehouse and installed by certified installers at William S. Middleton Memorial Veterans Hospital, 2500 Overlook Terrace, Madison, WI no later than September 14, 2020. Work is to be performed between 7:00am 6:00pm Monday through Friday, except when approved in advance by the Contracting Officer. Trades are to conduct themselves in a professional manner. Respect for patient care and privacy to be always maintained. Installation and delivery are to be coordinated with the interior designer. GENERAL REQUIREMENTS: Project Management: This project requires the vendor to have a project manager to oversee and coordinate the project with the interior designer. Coordination of the installers start & stop times are required, as well as coordination for VA staff to demo existing millwork before each are is installed. The project manager will create a detailed schedule for each area installed. Each desk install will need to be coordinated in advance so we can let staff know of the downtime, and to create a separate area for check in at each Clinic location. Only one location will be done at one time to create less disruption to the staff & Veterans. GEMS: The Wm. S. Middleton Memorial Veterans Hospital has a Green Environmental Management System (GEMS). The facility is committed to protecting and improving the environment. Our goals are: Taking leadership role in environmental stewardship by providing a clean and safe environment in our community. Conserving natural resources and supporting their sustainability by upgrading our environmental system. Reducing the use of hazardous products and the generation of wastes Purchasing renewable, reusable, recyclable and recycled products Working to constantly improve the immediate and long-term environmental impacts of the products, services and processes used by the Madison VA Healthcare System. Waste disposal in landfills shall be minimized to the greatest extent possible. Of the inevitable waste that is generated, as much as possible shall be salvaged, recycled, or reused. At a minimum, the following waste categories shall be diverted from landfills: clean dimensional wood, palette wood, cardboard, and paper packaging. Vendor shall provide the VA with a detailed report of recycled materials prior to the final invoice. Vendor shall assign a specific area to facilitate separation of materials for reuse, salvage, recycling, and return. Such areas are to be kept neat and clean at all times. SALIENT CHARACTERISTICS a. Common Nomenclature (commercial description): Modular cabintry, walls and countertops throughout our remodeled first floor. Modular, demountable walls to sit on top of finished flooring. Product shall have a 10 year warranty on the construction of the walls. b. Kind of material (i.e., type, grade, alternatives, etc.): Modular cabintry and wall system utililzing the same 3D wood laminate that can be field modified or adjusted. This product must be 99% resusable, and 96% recyclable at the end of life and use. The VA mandates a green purchasing policy requiring medical centers to purchase products utilizing 30% recycled content. The drawers have a soft close and will shut itself. Cabinets and are made with an MDF board that s 48 pounds per cubic foot density, industrial grade. Base cabinets are constructed of ¾-inch thick MDF sides, tops and bottoms. Overhead cabinets are constructed of ¾ inch thick MDF sides, tops and bottoms. Cabinets are assembled with metal bolt and brass inserts. The countertop core is a 1-inch thick medium density fiberboard; drawer bodies are 5/8 thick medium density fiberboard. The sides and fronts are a 3D laminate that s .012 to .020 inch nominal thickness. The standard edge detail for the 3D laminate is a self-edge eased at 3mm. All 4 edges are finished. Interior door and drawer fronts shall be finished with white melamine. Hinges shall be concealed 107 degree swing hinge, with two hinges per door up to 36 , 3 hinges per door on extended height cabinets. Drawers are standard equipped with Tandembox B,D,M or N premium series full extension drawer system dynamic load capacity of 75lbs and 110lbs. All file drawers are standard equipped with a 6mm aluminum file bar. Standard pulls are a Bow Pull #117.31.632 from Hafele and a Rod Pull #305128195 from Richelieu. Shelf supports shall be flattened self support pins of 5-millimeter nickel. Shelves are adjustable in all cabinets. Grommets are 3-inch black plastic. Cabinet shelves are ¾ inch thick MDF and finished on all sides. Cabinet backs are ¾ inch thick MDF laminated with white thermally fused melamine on interior side for 3D laminate cabinetry. The 3D laminate is heat formed around 5 surfaces to create a seamless self-edge that won t chip, crack or delaminate. Walls shall be constructed of aluminum frames and include mouldar blocking. Wall construction shall include low profile base track with levelers to accommodate 1.5 adjustment. Base shall meet the floor with carpet grippers or two sided tape, pending location and flooring type. Base trim shall consist of santoprene material to form a sound and light seal. Walls shall have face-mount tiles with the same material and finish characteristics as cabinetry. Face tiles shall be adhered to aluminum frame with a polycarbonate clip system, factory installed. Walls shall be parametric in size and universally and horizontally support all new and legacy furniture, fixtures and equipment. Each wall side performs and is aesthetically independent of the other. Tiles can be on or off-module. Power can be integrated inside and be UL listed. Wall cavities are accessible for maintaining, replacing or adding technology, plumbing or electrical outlets. c. Electrical Data (i.e., 110 v, 60 Hz, 3 Ph, etc.): Electrical needed for all the clinic desks & reception desks for data/electrical needs. d. Dimensions, size, capacity: e. Principles of operation: Modular cabinets, walls,storage and countertops needed for our Clinics & main Information desk of our first floor. The specialty modular cabinets help assist with the patient needs. f. Restrictive environmental conditions: N/A g. Intended use: Modular cabinets, walls,storage and countertops needed for our Clinics & main Information desk of our first floor. Walls to be used to divide reception waiting areas from public corridors h. Equipment with which the item is used with (N/A if not applicable): N/A i. End item application: To provide specific cabinetry that will be used in Patient & staff areas to make efficient to assist patients. j. Original equipment manufacturer part number: N/A k.
