Description
Scope of work: Replacement of Mental Health Unit Door Locks Replace 45 existing door locks with Sargent Anti-Ligature Resistant Door Lock 70-8205BHW-US32D-LH. This lock is the approved lock for our facility s mental health unit. Modification is due to a widespread, moderate risk Joint Commission finding. The mortise part of this lock is interchangeable with the current locks and doors throughout facility, the quality of Sargent is proven to last with minimal maintenance cost. Other comparable locks will not be acceptable. VA Engineering will review products before purchasing. Environment of work: Locks are all located on 5th floor mental health unit. This work requires (2) workers to be together at all time when working on unit. Workers must make sure tools are not left unattended, keep conscious of their environment and surroundings at all time. A VA employee will always accompany contracted employees. No work to be done unless unit staff is aware of contractors presents. Description of work: Work will consist of one door at a time, not multiple doors at one time. Unit staff will be responsible for determining, through engineering, sequence of doors to be done until all doors are completed. Remove existing door lock. Install new anti-ligature door lock per manufacturer instructions. Patch any holes left exposed from previous lock with VA supplied wood filler that will to some extent match existing door color. Existing cores will be used in new locks. Cleanliness: While working on unit, no debris can be left unattended at any time. Contractor is responsible for all cleaning (dust, wood chips, trash etc.). All tools and materials must always be accounted for. This is for the safety for all people on this unit. Staff will be notified when coming on to unit and when you are leaving unit. Engineering employee will assist with all coordination.
