- INTENT TO SOLE SOURCE: The Research service at the Jamaica Plain VA Medical Center/VA Boston Healthcare system located at 150 S Huntington Ave, Boston MA 02130 is proposing to sole source from Philips Medical System located at 22100 Bothell Everett Highway, PO Box 3003, Bothell WA 98041 for the purchase and installation of the Brand Name or Equal, Phillips Picture Archive and Communication System (PACS) Software and workstations with service plan. Description and Quantity of Supplies: The Scope of work involves the following specific tasks: Install the Philips PACS Clearstream 12.2. software with the capability of supporting less than 7.5K exams per year with the following: 3D VUE motion unlimited SW less than 50k exams per year. DBR WFM/VNA less than 25K exams per year. Install reading licenses with the following: Unlimited Reading Licenses less than 7.5K exams per year. 3D reading package for unlimited access less than 7.5K exams per year. Install workstations as follows: 3 HP Z4 Power Workstations (Note: One installed to support the Systems Administration requirement with the remaining two workstations to be set-up when the Neuroscience Building is completed). 3 Admin Display, 22 inch HP Color monitors (Note: One installed to support the Systems Administration requirement with the remaining two displays to be set-up when the Neuroscience Building is completed). 3 BARCO Color 6MP 6MP MDCC6430 +GC for V11/V12 (Noe; One installed to support the Systems Administration requirement with the remaining two to be set-up when the Neuroscience Building is completed). Professional Services - Install and Training as follows: 64 HCIS project management services. 120 HCIS technical implementation services. 3 applications consulting/32 hour blocks (Note: One installed to support the Systems Administration requirement with the remaining two to be set-up when the Neuroscience Building is completed). Custom Technical Advanced Training 1 Day Block. 4 Image network consulting and planning. SRSA SSL Remote Security Application CAD-0225. Service Agreement for Five (5) Years. Gray Market Prevention Language (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. This notice of intent to sole source is not a request for competitive quotes; however, any responsible source who believes it is capable of meeting this requirement may submit a capability statement to the contracting officer later than Monday, July 13, 2020 at 11:00 AM ET. A determination not to compete the proposed requirement based upon the responses to this notice is solely within the discretion of the Government.
