(NOTE: this same question was asked in amendment 2 and amendment 3 with the answer in Amend 3 stating that email submission is acceptable and the answer in amend 2 stating that submissions shall be submitted via hard copy to David Malm)- note that email submission would be preferred if possible Proposals could be submitted via e-mail however samples will need to be shipped. Please clarify if a physical sample frame kit is required with proposal submission or if only frame selection charts are to be submitted? (NOTE: the solicitation appears to state that only frame selection charts are required but in response to a question in Amend 3 it states that physical frame selection and chart required to be mailed ) Yes, both frame selection charts and a physical sample of frame selection will be required. Can VA please take another look at the estimated quantity of GLASS bifocals? The quantity listed of 25,484 is much higher than what would be expected and much higher than the more realistic amounts listed for SV glass (1620) and Progressive glass (2389). An error of this margin could lead to significant inequities in bidding amongst the contractors submitting bids. This is an error and the numbers would be much lower than 25,484. See revised Bid Schedule In the Bid Schedule the volumes for Bifocals is consistent from 0001AB to 0004AB but in Year 5 (0005) the quantities that had been used for Bifocals are used for Single Vision and the quantities used for Bifocals is different than what had been used in Years 1 to Year 4. Can VA please correct? Corrected, see attached Bid Schedule for revised quantities. Thank you for bringing this to our attention Are optician services currently being provided by VA employed opticians, opticians provided by the current contractor, or a mixture of both? Under this new contract will the contractor be providing all of the opticians and if so, will the contractor be able to bill the VA for optician services? Some sites have VA employed opticians; some sites have contracted opticians. VCS will be setting up a contract in conjunction with this contract for optical shops at sites that desire this service. Optician pay will not be a part of this contract. All Optician CLINS have been removed from the Bid Schedule Can VA please clarify what information contractors are expected to provide in CLIN006 to CLIN 0010 (Optician Average Annual Wage)? In answers provided in Amendment 3 it was stated by VA that these will be established with a separate VCS contract. Are we therefore supposed to leave these CLINS blank until discuss with VCS take place on a contract? If not blank, could VA please provide an example of the information expected if no information other than the number of opticians required for each location (e.g. 2 for Minneapolis) is being provided in the SOW? Yes, this will be negotiated in the VCS contract. See Revised Bid Schedule attached to this amendment A question was asked in Amend 2 if VA could add anti-reflective coating and oversize lens blanks to the bid schedule and the response was yes, please do . May we suggest that VA add these o the bid schedule with appropriate (estimated) quantities so that all contractors are bidding on the same items/quantities? See Revised Bid Schedule attached to this amendment. In response to question 11 in Amend 3, VA stated that if noted items are duplicated, please make a note in the bid schedule to reflect the duplication . May we suggest that VA correct items that may be duplicates as asking contractors to note them in their bids could lead to inconsistencies among bids provided by contractors? (NOTE: In our opinion, because sections 0001AE to 0005AE, Hi-Index lenses, are broken down by lens style (SV, BF, TF, Prog) they are the only items that could be confused as being duplicates. May we suggest that Hi-Index lenses just be listed as an add-on without reference to a specific lens material?
