- SOURCES SOUGHT NOTICE LAUNDRY SERVICES COATESVILLE VAMC This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No quotes are being requested or accepted at this time with this notice. The intent of this Sources Sought Notice is to identify potential Small Businesses especially any SDVOSB/VOSB any and other business size offerors capable of providing Bibliographic Management Software Services as listed in the Statement of Need (See Below). The North American Industry Classification System (NAICS) code being considered for this procurement is 519130, all other support services Code (PSC) is S299, other housekeeping services. This is for the VA Healthcare Center in Wilmington, DE 19805 Below is a draft copy of the Statement of Work (SOW). Responses to this notice should include the following: 1. Company name 2. Data Universal Numbering System (DUNS) number 3. Company s address, 4. Point of Contact information (i.e. title, phone number and email address) 5. Company s capability to meet this requirement and any pertinent information which demonstrates the company s ability to meet the above requirements. 6. GSA Federal Supply Schedule contract number, if within scope of this effort (if applicable). Company s type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Women Owned, Small Disadvantaged, etc. If applicable, Service-Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) must be verified in www.VetBiz.gov , and be able to meet the limitation on subcontracting in accordance with 13 CFR 125.6; if applicable. Offeror must provide the above requested information. After review of the responses to this sources sought notice, a solicitation announcement may be published on the FBO website in the near future. Responses to this sources sought announcement are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the forthcoming SUPPLIES OR SERVICES AND PRICES/COSTS Contractor shall furnish all labor, supervision, management support, transportation, supplies, equipment and materials necessary for accomplishment of complete weekly and monthly, on-site shredding services of medical, financial and other confidential records for the VA Medical Center, Wilmington, DE, one VA annex and 5 Community based Outpatient Clinics (CBOC). The contractor will provide a minimum of 25-95 gallon and 70-40 locked containers to be placed at Location of containers will be as directed be the VA Medical Center. Contractor will provide 2-95 Gallon locked containers for each CBOC and VA annex. Locations are the Wilmington VAMC main hospital (8 floors and out-buildings throughout the campus), 5 CBOCs: Atlantic County CBOC, Northfield, NJ. 08225, Cumberland County CBOC, Vineland, NJ. 08360, Cape May County CBOC, Cape May, NJ. 08204, Sussex County CBOC, 21748, Georgetown, DE. 19947, Kent County CBOC, Dover, DE. 19901. VA annex, Greenville DE. Three (3) keys shall be provided to the Wilmington VAMC COR.in accordance with the following contract terms, conditions and requirements. Items to be shredded may include but are not limited to paper, folders, x ray file jackets, patient ID bracelets, patient ID cards, CD s, VHS tapes and self-adhering prescription labels. Contract shall span from October 1, 2020 through September 30, 2021 with four (4) one -year option periods. Qty 70) DESCRIPTION/SPECIFICATIONS/WORK STATEMENT The contractor will follow VA Directive 6371, Destruction of paper records, references final destruction to be temporary paper records are pulped, macerated, shredded or otherwise destroyed to a degree that definitively ensures that they are not readable or can be reconstructed to any degree. BAA DO it Environmental Management service will provide someone to escort the contractor to witness that the sensitive materials have been shredded and all items are unreadable per VA directive 6371. Contractor shall execute the terms of this contract in a manner that will safeguard the confidentiality of all documents and information exposed at any of the facilities. The following parameters will be applicable. The chosen shredder device must have a crosscutting capability which produces particles that are ¾ X 2 1/2 millimeters in size or that will pulverize/disintegrate paper material using disintegrator devices with a 6 inch security screen. (Reference NSA Disintegrator Evaluated Products List). Contractor shall provide locking containers in the following size(s), 95 gallon 46.5 h x 26.38 w x 33.62 d, and 40 h x 19 w x 19 d for the collection of the documents intended for shredding by the Wilmington VAMC main hospital (8 floors and out buildings throughout the campus), 5 CBOCs (located in Vineland, Northfield, and Cape May NJ and Georgetown and Dover DE), and Annex (located in Greenville, DE). Three keys shall be provided to the Wilmington VAMC. Quantities and locations of collection bins may be adjusted to meet VA requirements and changing workload. Contractor shall provide, 70 40 h x 19 w x 19 d containers, and 25 95 gallon containers to the main hospital and 2 95 gallon container to each of the 5 CBOCs and 2 95 gallon containers to the Annex. Main hospital shredding is to be done once per week, Annex once per week, and CBOCs will be conducted once per month. Contractor shall be conducted bi-annual mass shredding. Contractor shall be responsible for collecting the locked containers from the predetermined locations at each facility, by emptying the smaller containers into a larger lockable collection bin used to transport all materials to the onsite shredding location. Contractor shall ensure the return of emptied containers to those same locations after the shredding process is completed. Contractor shall be responsible for disposing by recycling all shredded material off site at own expense. Contractor personnel involved in the actual handling and shredding of documents at any facility shall have a background check completed. Contractor shall allow only individuals aged 18 or over, who have appropriate contractor identification, to assist with the shredding/removal. Contractor shall provide a written statement of assurance that all documents intended for shredding will be completely destroyed and unreadable utilizing an on site, crosscut shredding device. Contractor shall provide written assurance that all data is destroyed on site, that nothing is removed from site unshredded. Contractor personnel involved in the actual handling and shredding of documents shall wear a contractor-provided, clearly readable identification badge that at a minimum-states, the name of the employee and the company. Contractor personnel s clothes are to be clean and maintained in good repair. Contractor shall be required to interrupt their work at any time so as not to interfere with the normal functioning at any facility, including utility services, fire protection systems, passing of facility patients, personnel, equipment and carts. Contractor shall be provided a written schedule of the days and times of anticipated performance in this contract. Contractor is required to perform shredding/pick-up on day agreeable to both Wilmington VAMC and the Contractor. Any changes to the schedule shall be approved in advance of the contractor visiting the facility. The contractor shall schedule the times during normal business hours, which are between 8:00 am and 4:30 pm, federal holidays excluded. A Wilmington VAMC Contracting Officers Representative (COR) shall conduct an initial orientation to the facilities; however, thereafter the Contractor shall be responsible for conducting orientations to new Contractor employees. This will not relieve the Contractor s obligation to provide documentation of trustworthiness of new employees. Contractor s vehicles used to perform this contract, shall be locked and the keys removed when not in use. Vehicle(s) shall be identified and parked in a predetermined location agreeable to both Wilmington VAMC and the Contractor. Contractor shall perform all work in accordance with the guidelines of Federal, State and local ordinances. All safety practices prescribed by these agencies shall be followed. When applicable, Material Safety Data Sheets (MSDS) are required to be submitted to the Wilmington VAMC. Contractor shall have a quality control program to assure the requirements of this contract are provided as specified. Contractor shall instruct contractor personnel in utilities conservation practices. Contractor shall be bonded or be covered for Error and Omissions Insurance in the amount of at least $500,000.00. Contractor agrees to provide containers as outlined above under heading of Base Period. The collection bins must be secure from unauthorized access, i.e. reaching through the opening to retrieve a document, by-passing the locking mechanism with a devise other than a key. Contractor shall place collection containers in the areas identified by the Wilmington VAMC.
