- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this source sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified vendors interested and capable of providing the services described below. Documentation of technical expertise must contain sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) Company name (b) Point of contact (c) Phone and email (d) Type of small business, e.g. Services Disabled Veteran Owned Small Business, Veteran-owned Small Business The Northern AZ VA Health Care System located in Prescott, AZ, 500 Highway 89 North, Prescott, AZ 86313. The requirement is for an Eye Fitting Technician. CONTRACTOR NEEDS TO BE ABLE TO MEET THE NEED IN PRESCOTT AZ The contractor shall provide 40 hours of professional services per week. Routine work hours for Eye Fitting services could range from 8:00 am to 4:30 pm Monday through Friday, except for Federal Holidays. Work hours are at the discretion of the S&D Service Line Chief and S&D Administrative Officer. Federal Holidays: New Year s Day Birthday of Martin Luther King, Jr. Washington s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day FITTING SERVICE (DISPENSING OPTICIANS): The contractor shall supply one Licensed Dispensing Optician (DO) at Northern AZ VA Health Care System (NAVHCS). The duties of the DO include measuring for frames, review of the prescription, and assistance in frame selection Pricing shall remained unchanged from contract pricing for facilities as listed in Prosthetics eyeglass contract. The DO shall maintain a mounted display of the sample frames within the Optical Shop and work with Prosthetics Services and their eyeglass and lab contractors. The VA will provide a space within NAVHCS Medical Center for an Optical Shop to measure and fit eligible Veterans entitled to government provided eyewear. The location provided by VA for the DO to perform measuring and fitting will be determined by the facilities chief, or their designee. There is a minimum guarantee of space location for Optical Shop(s) and this is solely at the discretion of the facilities space availability and needs for DO. When an Optical Shop is selected, the facility will have sole discretion and retains the unilateral right to select one of the Contractors under contract with NAVAHCS, and the selected Contractor shall be the sole provider of eyeglass fittings services at NAVHCS, further codified in a contract modification when a selection is made. The contractor shall supply at least one licensed Optician (DO) to NAVHCS Medical Center for the Optical Shop, for measuring and fittings all Veteran patient prescriptions provided by the Doctors in the NAVHCS, Eye Clinic. The Optician must also be able to verify eye glass prescription specifications provided by the NAVHCS Eye Clinic. All opticians will have access to Protected Healthcare Information (PHI) and to the VA Information System (VISTA) and shall be required to submit information for a Background Investigation and security clearance. This and additional requirements concerning use of VA information systems are in Security Requirements, of this contract. The DO will be required to use the VA NAVAHCS consult system to enter, edit, and update patient consults. The DO will fit/adjust and/or repair beneficiary eyeglasses. They will work alongside other VA clinicians, and administrative staff in placing and processing all orders. The DO shall maintain a mounted display of the sample frames within the Optical Shop provided by Prosthetics Services and their eyeglass and lab contractors. ACCESS TO VA INFORMATION AND VA INFORMATION SYSTEMS Due to the threat of data breach, compromise or loss of information that resides on either VA-owned or contractor-owned systems, and to comply with Federal laws and regulations, VA has developed an Information and Information Technology Security clause to be used when VA sensitive information is accessed, used, stored, generated, transmitted, or exchanged by and between VA and a contractor, subcontractor or a third party in any format (e.g., paper, microfiche, electronic or magnetic portable media). In solicitations and contracts where VA Sensitive Information or Information Technology will be accessed or utilized, the CO shall insert the clause found at 852.273-75, Security. Contractor/subcontractor shall request logical (technical) or physical access to VA information and VA information systems for their employees, subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures. PAYMENTS Invoices: Invoices for services shall include: name and address of agency monthly reports on the number of patients seen Monthly report of eyeglasses returned for correction with reason for return. description of the services Contract: Base Year + 4 Option Years GENERAL REQUIREMENTS STANDARDS OF PERFORMANCE: The contractor shall supply one Licensed Dispensing Optician (DO) at Northern AZ VA Health Care System (NAVHCS). DIRECT DELIVERY TO VETERANS: In the event NAVHCS Medical Center activity or facility requests delivery directly to the Veteran the VA will not need confirmation of deliveries and does not desire any additional warranty in relation to this type of delivery. The contractor agrees to make fitting adjustments to veterans at no additional cost to the Government. These conditions do not apply to adjustments incidental to the wearing of eyeglasses, adjustments required by physical change of the wearer, or where there is evidence of deliberate misuse or alteration by anyone other than the contractor. If eyeglasses are returned to the contractor for correction of a problem due to contractor error, the corrected glasses shall be returned to the VA facility within three (3) calendar days of receipt of the defective glasses. The contractor shall immediately notify the S&D COR, Margaret E. Lancaster or Contracting Officer in the event of a product recall. removal, required adjustment, required modification or substitutions that is suggested or mandated by the contractor, distributor, manufacturer, or any regulatory or official agency. The information that shall be included in this notification includes: A complete item description, identification. Administrative identification data including contract number, order numbers, and order date. Reasons for recall, remove, adjustment, or modification. Instructions for appropriate corrective action. IMPORTANT INFORMATION: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this source sought synopsis request. This notice does not commit the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. This is open to all sizes of business. There is anticipated sub-contracting opportunity for this procurement. Must be able to meet the requirement in the Prescott area.
