Description
FY20 NATR WELL CAPPING NATR A. These wells have been abandoned and are no long in use. State regulations require all abandoned wells to be properly plugged (capped) by an approved licensed contractor to prevent contamination and to eliminate a safety hazard. After the well is plugged, the Contractor shall provide a copy of the Decommissioning Form to the appropriate state agency, the Natural Resources Conservation Service, and to the project COR. B. A safety plan needs to be prepared and submitted to the COR before work can begin. The Safety Plan shall be a written document that describes the process for identifying the physical and health hazards that could harm workers, procedures to prevent accidents, and steps to take when accidents occur. The written safety plan is your blueprint for keeping workers safe. The plan shall compile the list of activities and what hazards exist and what safety equipment will be used to prevent accidents from occurring. The plan shall include 24-hour emergency contact information. C. Any disturbed areas shall be regraded, seeded, and straw placed to prevent erosion per US DOT FHWA Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP-14). D. If this work is to be performed under a single contract, the contractor needs to consider that this work will occur in three (3) states, with each state not only having slightly different requirements, but also the requirement of the contractor being licensed by each states Environmental Department. Please see statement of work for more details.
