Page 1 of 3 The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with FIDELITY CONTRACT FLOORING LLC. SDVOSB to provide replacement and installation of Flooring in the Acupuncture Rooms 6488 and 6493 area for the Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 238330 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for Maintenance for MED GAS COMPRESSOR equipment for the Jesse Brown VA Federal Health Care Center to the contracting officer NLT 11:00AM CST, 10 Aug 2020. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with Doctor Oxygen STATEMENT OF WORK Transaction#: 537-20-4-6072-0817 Scope: The contractor shall provide all labor, travel, supervision, materials and necessary equipment to install Luxury Vinyl Tile (LVT) on the 6th floor of the Damen Pavilion, Rooms 6488 and 6493 of the Jesse Brown VA Medical Center in Chicago, IL. LVT installation will include removal of the existing VCT tile flooring, floor preparation, and installation of the LVT, ADA compliant transitions and 4 rubber wall base. Contractor will move all furniture and equipment to one side of the room, complete the removal of the old flooring and install of the new flooring prior to moving the equipment to the newly completed side and completing the rest of the flooring work. All work shall be performed according to manufacturer s specifications and industry standards. QUALITY ASSURANCE: Installer Qualifications: A company specializing in flooring installation with minimum three (3) years experience and employs experienced flooring installers who have retained, and currently hold, an INSTALL Certification, or a certification from a comparable certification program. 1. Installers to be certified by INSTALL or a comparable certification program with the following minimum criteria: a. US Department of Labor approved four (4) year apprenticeship program, 160 hours a year. b. Career long training. c. Manufacturer endorsed training. d. Fundamental journeyman skills certification. Specific Tasks/Deliverables. Install Resilient Tile Flooring: Task 1: Moisture test of concrete slab. Contractor will conduct a moisture vapor emission rate using calcium chloride (ASTM F 1869) and/or percent internal relative humidity (ASTM F 2170) tests must be conducted in strict accordance with the most current edition of the ASTM test method. Allowable maximum moisture limits for the installation of the LVT must meet manufacturer s requirements. Task 2: Contractor will relocate all furniture and equipment to one side of the room. Task 3: Removal of Existing Vinyl Composite Tiles. Contractor will remove the existing flooring and discard in contractor provided dumpster. Task 4: Floor Preparation. Contractor shall ensure subfloors/underlayments shall be dry, clean and smooth. The floor shall be free of dust, solvents, varnish, paint, wax, oil, grease, and residual adhesives. Contractor will smooth, level and prepare all flooring surfaces to ensure new flooring adheres well to the floor. Whenever sanding, be certain the work site is well ventilated, and maintain negative air pressure within work area utilizing HEPA-equipped air filtration units per the ICRA. All power sanding tools must be equipped with dust collectors. Sticky walk off mats shall be supplied and utilized at all interior entry/exit locations of the construction space by the contractor. STATEMENT OF WORK Transaction#: 537-20-4-6072-0817 e. Task 5: Installation of the LVT. Contractor shall install the LVT planking, Flexco 1/8 Gauge, 28 mil wear layer, 4 x 36 LVT Planks Natural Elements Wood, Finish: Caramel 637 with a Bamboo pattern or approved equal in an offset pattern. Glue down for high traffic, durable applications. The material and adhesive must be acclimated at room temperature for a minimum of 48 hours before starting installation. Installation of both rooms shall take no more than 1 work week from Monday to Friday after normal business hours of 4:30pm to 7:00am. i. Resilient Tile Salient Characteristics: Floor tile: Solid vinyl tile which meets the following minimum standards of equal or better: 1/8 gauge, 4 x 36 vinyl plank Urethane ceramic bead finish 28 mil wear layer 100% pure virgin material ASTM F1700 Class III Type B Flammability: ASTM E648 (NFPA 253) >.45W/cm2, Class I Smoke Density: ASTM E662 Passes, .6 20 Year Limited Commercial Warranty Made in the USA ii. Area to be installed: Pavilion ROOM # SQUARE FOOTAGE Damen Pavilion 6488 281 Damen Pavilion 6493 221 iii. Install approximately 502 SF of LVT and approximately 138.3 LF of wall base. iv. Final color of flooring and wall base will be approved by Jesse Brown VA Interior Designer. v. Follow all flooring and wall base per manufactures installation requirements. Notify COR if any issues or concerns prior to installation.2 vi. Dispose of all floor preparation material and construction debris in accordance with Federal, State and local environmental requirements. vii. All debris (garbage and trash) needs to be covered at all times while it is in transit through hospital hallways to and from the dumpster. Dumpster needs to be covered at all times and emptied as needed. It is contractor s responsibility to keep the area surrounding your dumpster clean at all times. viii. Protect all existing electrical, and data outlets and any supporting equipment from damage during preparation work. STATEMENT OF WORK Transaction#: 537-20-4-6072-0817 ix. Protect all existing drainage structures from debris and sedimentation. x. Maintain temporary infection control enclosure during demolition and construction. xi. Deconstruct containment barriers, if any, remove all debris, and clean affected areas once project is complete. xii. Upon completion of the project, the contractor shall deliver all work completed and undamaged. Any damage that may be caused by the contractor or his work crews or subcontractors to any existing structure, grounds and utilities shall be repaired or replaced to the original conditions. xiii. Any deviation from this scope of work requires the express authorization of the Contracting Officer. The Contracting Officer Representative (COR) does not hold the authority to authorize changes to this scope of work. Task 6: Installation of Rubber Base. Contractor shall furnish and install a Rubber 4 wall base with standard toe. Color TBD. Remove existing wall base. Rubber Base Installation: All walls must be clean, smooth, flat and dry. The surface must be free of all dust, loose particles, solvents, paint, grease, oil, wax, alkali, sealing/curing compounds, old adhesive, and any other foreign material, which could affect installation. Remove existing adhesive mechanically do not use chemical adhesive removers or solvents. Fill all depressions, cracks, and other surface irregularities with a good quality patching compound. SBR Rubber Base salient characteristics: 1/8"" (3.2 mm) thick by 4"" (101.6 mm) height. Cove Toe Style Color 171 Sandstone 100% thermoset (TS) rubber wall base that is PVC free. Phthalates Free FloorScore® Certified Wall base is manufactured in the USA Warranty - Limited 2-year warranty. STATEMENT OF WORK Transaction#: 537-20-4-6072-0817 Task 7: Contractor will provide a receptacle for the old flooring. The old VCT will be removed from the VA premises by the contractor. Task 8: Contractor will supply ADA compliant transitions where necessary. Hours of Operation. Normal hours of coverage are Monday through Friday from 8:00 am to 4:30 pm CST, excluding holidays. All service/repairs will be performed after normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Place of Performance. Jesse Brown VA Medical Center Damen Ave Chicago, IL 60612-3728 Period of Performance. All work to be completed within 30 days after award of contract. Contractor shall warrant services against defects of workmanship under normal use for one year from date of installation.
