- AMENDED 8/10/20 PRICE SCHEDULE QUANTITY CHANGED Services Required: The Department of Veteran Affairs Pacific Health Care System (VAPIHCS) through the Health Care for Homeless Veterans (HCHV) Emergency Contract Housing program provides care, treatment and rehabilitative services to homeless veterans in the Pacific Region. The services provided to eligible homeless veterans include outreach, case management and housing services. It is the purpose of this program to provide these veterans a safe residential setting for a temporary period while addressing their immediate needs by accessing appropriate resources and supports with the goal to obtain permanent housing and end their homelessness. To provide the care, treatment and rehabilitative services required the VA- HCHV program to contract with agencies and/or organizations (Contractors) to stabilize the veterans� needed medical, mental health, substance abuse and other psychosocial problems so the veteran can transition into other appropriate transitional or permanent housing.� Since these veterans may have co-morbid conditions which may include serious mental illnesses, substance abuse disorders, and other serious medical conditions the Contractor is required to possess an understanding of these challenges and general skills to support the veterans to address these issues. � Locations: Hilo, Hawaii���������������� five (5) beds per day for male Veterans Central Oahu: � ����������� five (5) beds per day for male Veterans over the age of 55 Lihue, Kauai: ������������� five (5) beds per day for male Veterans Mangilao, Guam: �������� five (5) beds per day for male Veterans This soliciation is being conducted under the authority of FAR Parts 12 and 13 and VAAR 873.
