Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart  12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C25720Q1047 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-08. The NAICS Code for this request for quotes is 325413 (1,250 Employees). No set-aside is used for this solicitation. Items are to be delivered to the VA North Texas Healthcare System Laboratory at 4500 S. Lancaster Rd. Dallas, Texas 75216. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria are technical acceptability and price. Under technical includes the ability to provide all of the requested items in accordance with the Statement of Work, provide a letter of authorization (if not the manufacturer) and a description of equal items, and have, at minimum, a Satisfactory past performance rating.
