- This combined synopsis and solicitation�is hereby amended to extend the�closing date from 08/11/2020 to 08/17/2020, and attached the Indian Health Service, Buy Indian Act, Indian Firm Representation Form. Respondent shall submit a completed copy of the Indian Firm Representation Form to the Government by the closing due date and time. All other terms and conditions remain the same. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71320Q00035 has been issued as a Request For Quotation for a firm fixed price commercial items supply contract for custodial supplies located at Salem, OR at one lump sum price for items on the price schedule. The performance period includes a one-year Base and 4 one-year options. The required delivery date is August 13, 2025 including option terms. The delivery location and place of acceptance is Western Oregon Service Unit aka Chemawa Indian Health Center, 3750 Chemawa Rd NE Salem, OR 97305, F.O.B. Destination.
