- Sources Sought: This is a Sources Sought announcement only. This is NOT a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621610, Home Health Care Services, Small Business Size Standard $15 Million. The Network Contracting Office (VISN 2) is seeking potential qualified Contractors that meet Joint Commission Standards to provide Durable Medical Equipment (DME) services throughout the VISN 2 Upstate New York VA Healthcare Network catchment areas of Albany, Bath, Buffalo, Canandaigua, Rochester, and Syracuse. Services to include environmental assessment, storage, delivery, set-up, repair, instruction-in-use, and/or pick-up of Government purchased items. The services shall also include the installation of grab bars, room air conditioners, and maintenance of feeding pumps. Some of the equipment that may need to be delivered and set-up includes, but is not limited to, feeding pumps, hospital beds, trapezes, patient lifts, mattresses, bathroom equipment, ambulation devices, room air conditioners, etc. Contractor is to provide a secure and well-lit, clean, dry, contained area, with adequate space for the storage of government owned, miscellaneous Veteran supplies & equipment. The storage area shall be climate controlled with a functional thermostat and heating and air conditioning system throughout the year. A temperature range of between 50 degrees Fahrenheit and 75 degrees Fahrenheit and 55% or less relative humidity shall be maintained at all times. Responses to this Sources Sought Notice must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website, if available. How long has your company existed? (2) Type of business such as: Service-Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a) Small Business, HUB Zone Small Business, Women Owned Small Business, Small Business, or Large Business. (3) Does your company currently have capabilities to provide the required services to all catchment areas? The following catchment areas are anticipated. Please address each item separately with yes/no and any necessary additional information to support your capability. a. Albany area b. Bath area c. Buffalo area d. Canandaigua area e. Rochester area f. Syracuse area (4) Does your company currently have capability to receive and store DME items? If no mechanism is currently in place, what time frame would you need to acquire the warehouses? The following catchment areas/sq. footage estimates are approximated below. Please address each item separately with yes/no and any necessary additional information to support your capability. a. Albany - 2,000 sq.ft b. Bath - 800 sq.ft c. Buffalo - 2,000 sq.ft d. Canandaigua - 800 sq.ft e. Rochester - 800 sq.ft f. Syracuse - 2,000 sq.ft (5) Will your company be a joint venture with another company? (6) Are you Joint Commission accredited? (If not accredited, you will have to provide documentation on how you will meet the JC standards) (7) Do you have past experience with the Department of Veterans Affairs Durable Medical Equipment program? Equipment maintenance (feeding pumps) (12) Explain how you have the ability to do environmental assessments and meet patient education requirements. (13) For the purposes of market research, please provide an estimate of what you typically charge for: a. Routine delivery/setup/installation of DME equipment; b. Routine feeding pump maintenance; c.
