- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation. Quotations are being requested. Solicitation 12-52B1-20-Q-0072 is being issued as a Request for Quotation (RFQ), to provide Springs Valley Dam Maintenance services on the Hoosier National Forest, Orange and Crawford counties, Indiana. The North American Industry Classification System (NAICS) code is 238910 and the size standard is $15mil. �This is a 100% Small Business Set-Aside. Pricing and technical information will be submitted to:� Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421.� NO FAX QUOTES WILL BE ACCEPTED.� E-mail quotes are requested at roger.manning@usda.gov.� Service Wage Rates are applicable.� Quotes must include vendor's TIN (Tax Identification Number) and DUNS number.� Vendors must be registered in SAM (www.sam.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically, submit only paragraph (b) from the provisions and a DUNS number.
