Description
The combined synopsis/solicitation is hereby amedment to chage the set-aside from ISBEE to Total Small Business. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71320Q00036 has been issued as a Request For Quotation for a firm fixed price commercial items supply contract for Musical Instruments located at Salem, OR at one lump sum price for items on the price schedule. The procurement of musical instruments is in support of music therapy for adolescents at the Chemawa Indian Health Center. The group music therapy is an effective treatment for patients needing behavioral health services. �The required delivery date is August 27, 2020. The delivery location and place of acceptance is Western Oregon Service Unit (WOSU) aka Chemawa Indian Health Center, 3750 Chemawa Rd NE Salem, OR 97306, F.O.B. Destination. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-08 dated 07/14/2020. The provision at 52.212-1, Instructions to Offerors � Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation � Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price and past performance. Submission shall be in the schedule of items format (Attachment 1). The respondent shall enter the unit price and extended totals for each item listed on attachment 1. If applicable, enter �equal item� manufacturer and manufacturer item number. The price shall include necessary supervision, management, labor, transportation, equipment, materials, shipping and any other direct incidentals costs, overheard and profit, unless the cost is identified as separate priced contract line item.�In addition, respondent(s) must complete Attachment 2- Indian Representation Form and submit with respondent�s quotation. Award will be made to the responsible respondent pursuant to FAR subpart 9.1. Quotes are due by August 21, 2020, 12:30 PM, PT.
