- AMENDMENT 2 TO REQUEST FOR QUOTE 36C25820Q0369: This notice is an amendment to a Request for Quote (RFQ) posted on Contract Opportunities on 08/25/2020. The RFQ was inadvertently omitted from the 08/25/2020 posting. Amendment 1 was posted on 8/26/2020 to correct this error; however, it is not easily searchable because it has a different title than the original posting from 8/25/2020. This amendment now incorporates the RFQ document and has the same title as the original RFQ posting. The attached RFQ can be viewed in the Attachment/Links section of this amendment. The Department of Veterans Affairs, Network Contract Office (NCO) 22 - Gilbert is requesting quotes from contractors to provide Pharmacy Intravenous (IV) Cleanroom Cleaning, Disinfecting, and Application of Sporicidal Agents services. The required services have been expanded to multiple locations and are to be performed for the Phoenix, Southern Arizona, and Northern Arizona VA Healthcare Systems located in Phoenix, Tucson, and Prescott, Arizona. This requirement is a Firm Fixed Price (FFP) contract, and the contract period will be for a one (1) year period, with four (4) potential option years to be exercised at the option of the Government. Based on the results of market research under Sources Sought Notice 36C25820Q0190, posted on 05/08/2020 at https://beta.sam.gov/, an electronic solicitation is now being issued at https://beta.sam.gov/. The VA is setting-aside this solicitation for Small Business. Responses are due by 12:00 Noon Pacific Daylight Time September 8, 2020.
