- RESIDENTIAL HEALTH CARE FACILITIES SOLICITATION NO.: 86615620R00007 COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS____This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, �Streamlined Procedures for Evaluation and Solicitation for Commercial Items,� as supplemented with additional information included in this notice. This announcement constitutes the only solicitation being requested and a written solicitation document will not be issued. The solicitation number is 86615620R00007 and the solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-0 dated August 13, 2020. This requirement is a competitive full and open. The associated North American Industrial Classification System (NAICS) code for this procurement is 519130 �Internet Publishing and Broadcasting and Web Search Portals� with a small business size standard of 1,000 employees. The anticipated contract type is a Firm Fixed Price (FFP) contract type in accordance with the Statement of Work (SOW). The Office of Residential Health Care Facilities requires access to a quality of care-related risk management data analytics tool for residential care facilities that produces customizable reports. This tool will be available on a subscription basis for up to 125 users. The contractor shall provide access to a Web?based portal dashboard report system, including customizable dashboard homepage risk rating report featuring summarized data encompassing regulatory surveys, occupancy, re?hospitalization, enforcement actions, quality measures, staffing, special focus facility designation, abuse/neglect issues, candidacy for special focus, likelihood of adverse outcomes, and automated alert notifications for up to 5,000 facilities. The purpose of this Amendment (Amendment 0001) is to: (1) provide the government's responses to industry questions and (2) to revise the solicitation as follows:�� (1) The government's responses to industry questions are provided in the attached excel spreadsheet; (2) Pages 35, 59, 60, and 64 of the solicitation have been revised to replace the term ""Performance Work Statement"" with ""Statement of Work""; and (3) Pages 1, 2, 3, 58, and 63 of the solicitation have been revised to reflect the need for quoters to also submit pricing for the base and option periods on the ""Schedule of Supplies/Services"".� Note that the period for questions has closed; and all changes are highlighted in �yellow�.�
