- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-07, Effective Date: 08-31-2020. This solicitation is set-aside for Small Business. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380: Testing Laboratory, with a small business size standard of $15 million. The FSC/PSC is H199: Quality Control-Miscellaneous. The Department of Veterans Affairs (VA), VA Caribbean Healthcare System (VACHS), San Juan; Puerto Rico requires to perform analysis of water samples and laboratory water analysis to determine natural organic matter community through several locations of the water distribution network system in the facility in accordance with contact terms and condition established in the solicitation. Contract Period: This is a onetime, stand along contract with a maximum performance period of one year after receipt of official contract document. The following solicitation provisions apply to this acquisition: 1. FAR 52.212-1, Instructions to Offerors Commercial Items In accordance with FAR 13.106-2(b)(3) Evaluation of quotations or offers. The Contracting offices will conduct comparative evaluations of offers. Government will award a contract resulting from this solicitation to the responsible quoters whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: a. Company profile and capability statement comparable to the marketing package your company uses in their ordinary course of business. Please ensure the following information is included: point of contact information, DUNS Number, company business size and socioeconomic category such as: Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran- Owned Small Business (VOSB), Women-Owned Small Business, HUBZone Small Business, Small Business. b. Provide evidence of Laboratory Certification by a nationally recognized body as outline above. 3. Information of personnel record keeping insuring qualification technician is UpToDate. c. Past Performance Factor - An offeror past performance is important in determining its ability to successfully perform the services described in the PWS. The Government is interested in ensuring that the offeror has a successful history of recent and relevant past performance. To be considered relevant and recent, all references shall be similar in scope to the solicitation requirement, and services performed within the last three (3) years preceding the submission of this quote. d. A complete Contract price schedule attached to this solicitation. 2. FAR 52.212-3, Offerors Representations and Certifications Commercial Items Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. SECTION C - CONTRACT CLAUSES: Are attached to this announcement. Additional instructions to Quoters. 1. Read the entire solicitation notice and attachments carefully. 2. Ensure you meet all the solicitation requirements and submit a complete quotation. 3. Quotes shall be submitted in English. 4. Ensure you are registered in the System for Award Management (SAM) and your registration is active. 5. Ensure you complete FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, online or submit a hard copy with your quote. 7. Due date for all question submission is September 5. at 10:00 am. Eastern Time. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
