- Page 2 of 2 PLEASE READ THIS NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE PUBLISHED. The Department of Veterans Affairs (VA) Veterans Health Administration (VHA) intends to modify contract 36C25618D0133 with Carestream Health, Inc. to add additional equipment to this preventive maintenance and repair agreement. The original procurement was conducted under Federal Acquisition Regulation (FAR) Parts 12 and 13. The Veterans Health Care System of The Ozarks (VHSO) located in Fayetteville has a requirement for a contractor to provide all parts, labor, tools, materials, equipment, software, facilities, travel, transportation and supervision necessary to complete all work associated with performing preventive maintenance, and corrective or repair services, for medical imaging equipment in addition to the equipment currently covered by this contract. Based on market research, and previous acquisition history, it has been determined that Carestream Health, Inc. is the only vendor capable of providing all the required preventive maintenance and repair services in accordance with original equipment manufacturer requirements. This notice will be distributed solely through this website. Interested parties are responsible for monitoring this website to ensure they have the most up-to-date information about this acquisition. The North American Classification System Code is 811219, Other Electronic and Precision Equipment Repair and Maintenance. The Government intends to negotiate with only one source under the authority of FAR 6.302-1. Interested parties may identify their interests and capability to respond to this requirement to the attention of James Hunt, email address: james.hunt2@va.gov, no later than Tuesday, September 17, 2020 at 4:30pm (Central Time). Only emailed responses to james.hunt2@va.gov will be considered. Information received will be utilized solely for determining whether to conduct a competitive procurement. A determination not to compete this proposed acquisition based upon responses to this notice is at the sole discretion of the Government.
