- THE FOLLOWING UPDATES NOW PERTAIN TO THIS SOLICITATION:� 1. PLEASE SEE UPDATED PWS LABELED ""SEPT 2020"" (ATTACHED) FOR �REQUIREMENTS REGARDING THIS SOLICITATION.� SPECIFFICALLY, ONLY PAPER MATERIALS ARE NOW REQUIRED FOR SHREDDING/DESTRUCTION.� ALL OTHER REFERENCES TO ANY OTHER DEVICES, MATERIALS, DISC...ETC, ARE NO LONGER VALID FOR THIS SOLICITATION.�� 2.� a)�--Per 45 CFR 164.502(e)(2) and 164.504(e) compliance with current DODM 6025.18---Implementation of the HEALTH INSURANCE PORTABILITY and ACCOUNTABILITY ACT (HIPPA) Privacy Rule in DoD Health Care Programs, a Business Associate Agreement may be required and executed upon final decision of Procurement Officer at time of contract award. b)�-- 52.212-2 - Evaluation -- Commercial Items (Oct 2014) EVALUATION PROCEDURES: The government will award a contract resulting from this solicitation on the basis of a 'Lowest Price Technically Acceptable' (LPTA) approach.� An award will be made to the lowest evaluated offer that is technically acceptable. c)�-- 52.212-3 - Offerors Representations and Certifications - Commercial Items (Nov 2017) All offerors must submit�representations and certifications in accordance with this provision along with their offer. The North American�Industry Classification System (NAICS) code for this solicitation is 561990. 3. Questions and answers have been attached to this solicitation as of 9 Sept 2020.� See attached document labeled ""Q and�A PII Shredding_9 Sept 2020"" ----------------------------------------------------------------End of Update--------------------------------------------------------------------- This solicitation notice--(related notice FA286019RA021) for commercial services is prepared In�Accordance With (IAW) the format of�Federal Acquistion Regulation (FAR)�subpart 12.6 as supplemented with additional information included in this notice.� This annoucement constitutes the only solicitation.� The attached doucments within this solicitation references all instructions and requirement descriptions needed for this notice.� The DoD manual 6025.18 is for HIPPA references only. Solicitation number FA286020Q0031 is being issued as a Request For Quote (RFQ) using the simplifice acquistion procedures in accordance with FAR part 13 guidelines.� The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-09.� All responsible sources may submit an offer to be considered by the agency IAW the attached solicitation number previously referenced. This is a� firm-fixed price, base (12mos) plus four (4) option year contract.� Interested vendors must submit pricing�for all years within thier quote.� All questions must be submitted NLT 3:00PM(est) Tuesday, 8 Sep 2020.� �All final RFQs�are due NLT 3:00PM(est)�Friday, 11 Sep 2020.� All questions and quotes must be submitted in writing via email to the POC referenced in this solicitation.��
