N66604-20-Q-8028 Amendment 0002: This RFQ has been amended to revise the SOW, CDRLS, quantity of evaluations, and evaluation basis, as detailed below.� This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6-Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Request for Quotation (RFQ) number is N66604-20-Q-8028. The Naval Undersea Warfare Center Division Newport (NUWCDIVNPT) has a Firm Fixed Price requirement for services in accordance with Attachment #1 Statement of Work (SOW) and Exhibit A Contract Data Requirements List (CDRLs) for the following Medical Evaluation Services: Line Item # 0001 Description Medical Evaluation Services Quantity 10 Each Period of Performance 1 OCT 2020-30 SEP 2021 Line Item # 0002 Option 1 Description Medical Evaluation Services Quantity 10 Each Period of Performance 1 OCT 2021-30 SEP 2022 Line Item # 0003 Option 2 Description Medical Evaluation Services Quantity 10 Each Period of Performance 1 OCT 2022-30 SEP 2023 Line Item # 0004�Option 3 Description Medical Evaluation Services Quantity 10 Each Period of Performance 1 OCT 2023-30 SEP 2024 Line Item # 0005�Option 4 Description Medical Evaluation Services Quantity 10 Each Period of Performance 1 OCT 2024-30 SEP 2025 Requirements: Requirements: This procurement is solicited as a 100% Small Business Set-Aside, as concurred with by the NUWCDIVNPT Office of Small Business Program (OSBP). The North American Industry Classification System (NAICS) Code for this acquisition is 621330; the Small Business Size Standard is $8M. The required period of performance is listed in the table above; F.O.B. Destination is Newport, RI. Incorporated provisions and clauses are those in effect through the latest Federal Acquisition Circular. Defense Priorities and Allocations Systems (DPAS) rating is DO-C9. A Section 508 does not apply. The Government�s preferred method of payment is credit card.� If the offeror does not accept Mastercard or a surcharge is applicable, payment may be made through Wide Area Workflow (WAWF).� Award will be made to the offeror whose quote represents the lowest priced technically acceptable (LPTA) offer.� In order to be deemed technically acceptable, an offeror must provide the following: 1.� Assertion that the offeror proposes to perform this tasking utilizing board certified psychiatrist(s) or licensed clinical psychologist(s). 2.� Provide copy of current licenses and certifications. 3.� Provide three (3) past performance examples describing experience performing tasking similar to this requirement Statement of Work.� The past performance examples must be within three (3) years of the closing date of this solicitation.�� 4.� Provide address of location where the offeror will perform the evaluations.� The address must be within sixty (60) miles of NUWCDIVNPT (1176 Howell St., Newport, RI 02841), as calculated by a standard Google Maps search. 5.� The Offeror must state any potential Conflicts of Interest 6.� The Offeror must provide pricing for all base and option CLINs.� 7.� The Offeror must complete, sign, and submit Attachment 2 � FAR 52.204-24 with its quote. Additionally, the Government will consider past performance information in accordance with DFARS Provision 252.213-7000, where negative information within SPRS may render a quote being deemed technically unacceptable. The quotes must be submitted via email to the below address and must be received on or before Wednesday, 23 September 2020, 2:00PM EST.
