VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Original Date: 10/12/17 Revision 04 Date: 06/10/20 DESCRIPTION - 5 OPTOMETRY CHAIRS AND 4 COMPATIBLE STANDS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-03 published in the Federal Register on December 13, 2019. This solicitation is set-aside for SDVOSB participation only. The associated North American Industrial Classification System (NAICS) code for this procurement is 339115, with a small business size standard of 1000 employees. The FSC/PSC is 6540. The Tuscaloosa VA Medical Center is seeking to purchase 5 Optometry chairs and 4 compatible stands that are same as or equal to the following products: 1. Marco E-Z Tilt Ophthalmic Examination Chair (Catalog No. 1216) 2. Marco Deluxe Stand, Warm Grey (Catalog No. 1260 MG) Requesting this brand/model to match, fit, and integrate into TVAMC Optometry s existing systems, space, decor, and equipment. However, offerors of products that can meet the requirements of this SOW are welcome to compete. Contractor Shall: Provide the specified chairs and stands identified here within the SOW Provide full onsite installation, calibration, setup and end user training to ensure the equipment is fully operational without exceptions Provide phone and virtual support as needed Include installation with equipment costs Provide technical and end user manuals NOTE: Vendor s should clearly identify any of the aforementioned requirements that are not included in their proposal. Background: The Tuscaloosa VA Medical Center s COVID-19 related modernization efforts have identified an urgent need to replace five (5) Optometry Chairs and four (4) compatible Deluxe Stands supported by full service onsite installation, calibration, end user and Biomedical Training or agreeable training with follow-on phone support and virtual assistance as needed. Scope of Work: The contractor shall coordinate installation with the service immediately following contract award. Contractor shall bring awarded equipment and systems to the satisfaction of the government as intended by this requirement without additional or surprise costs and provide the onsite installation services necessary to maintain all manufacturer warrantees and guarantees. The contractor shall ensure the delivered equipment and or systems are properly calibrated and operate to or above manufacturer or industry standards. The contractor shall provide one project manager/point of contact for the government. Onsite service and support includes all activities associated with warrantee, guarantee, repair, services, and maintenance work and training necessary to maintain 99% operational status unless otherwise and exception is reasonable and accepted by the government. The scope of this requirement includes timely delivery of the specified systems to maximize discounts, installation and service, and consultations and training with modification options to ensure the Medical Centers expectation and requirements for access and high quality care provided in a safe work environment are achieved and sustained as related to this requirement. Trade in Values: Provide offer discount for 5 pre-owned chairs & 4 stands also to include 1 Zeiss FDT. Discount must be identified in the proposal and clearly identify discount amount, credit towards other equipment, and or other value including removal, shipping, and handling of used chairs and or other equipment. A proposed discount for traded in equipment must show the total cost adjustment considering the proposed equipment and services without the trade in to ensure the government clearly understands the value of the trade in with no surprises or undisclosed costs. The winning proposal requires the contractor to immediately work with Biomed and logistics to plan a proper transition of property according to agency standards. The government reserves the right to pay the full cost of the proposed equipment and services and cancel the trade in component at will without penalty. General Requirements: The installation of the Optometry chair AND stand delivery systems are all inclusive thus price break down for individualized tasks are not required. Proposals should include vendor expectations (if any) for site prep by the government to allow the contractor to effectively begin and complete installation and setup. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: price, technical speed of delivery credit for trade-in The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions.
