- Amendment 2 The purpose of this amendment is to revise various sections of SIR/RFP 693KA9-20-R-00013 as follows: Section B.4 Pricing Schedules From: B.4������ PRICING SCHEDULES note contractors may add rows for additional labor categories To: B.4����������� ����������� PRICING SCHEDULES Section L.14.2 Proposal Organization/Format paragraph (a) From: (a)The proposal must consist of two separate three-ring, loose leaf binders. One for the technical proposal and one for the price proposal. Each binder must be individually titled: VOLUME I -� TECHNICAL PROPOSAL and; VOLUME II� - PRICE PROPOSAL. (b)To aid in evaluation, all pages of each section must be appropriately numbered and identified with the date and the solicitation number to the extent practicable.� Each copy of the proposal must be labeled on the front identifying Offeror�s name, SIR number, and the proposal copy (e.g., Copy 1 of 2 and Copy 2 of 2). (c)Copy number l of the proposal package must contain the signed original of all documents requiring signature by the Offeror.� Use of reproductions of signed originals is authorized in all other copies of the proposal. (d)PRICING DATA MUST NOT BE INCLUDED IN VOLUME I.� IF ANY PRICING DATA IS INCLUDED IN VOLUME I IT WILL BE REMOVED AND DISREGARDED. (e)Page size must be 8-1/2 by 11 inches and must be printed on both sides with each side enumerated for the page count.� The text font must be twelve (12) point or larger.� When both sides of a sheet of paper contain material, it will be counted as two (2) pages.� Whenever practical, enclosed headings, subheadings, title, sequence of material and a number identification system must be used.� � To: (a)The proposal must be submitted electronically and consist of two separate volumes: VOLUME I - TECHNICAL PROPOSAL and; VOLUME II - PRICE PROPOSAL. (b)To aid in evaluation, all pages of each section must be appropriately numbered and identified with the date and the solicitation number to the extent practicable.� Each copy of the proposal must be labeled on the front identifying Offeror�s name and SIR number.� Each volume of the proposal package must contain the signed original of all documents requiring signature by the Offeror. (c)PRICING DATA MUST NOT BE INCLUDED IN VOLUME I.� IF ANY PRICING DATA IS INCLUDED IN VOLUME I IT WILL BE REMOVED AND DISREGARDED. (d) the offeror must use a Microsoft Office software compatible with the Microsoft Office 2016 software suite to submit the proposal.� The text font must be twelve (12) point or larger.� � Section L.14.3.1.2 Preparation Instructions. Tab (D) (2) is revised: From: (2)The offeror must submit a copy of the most recent past performance report prepared by Dun & Bradstreet.� The offeror may also describe any quality awards or certifications that indicate the offeror possesses a high-quality process for developing and producing the product or service required. Such awards or certifications include, for example, the Malcolm Baldridge Quality Award, other government quality awards, and private sector awards or certifications (e.g., the automobile industry's QS 9000, Sematech's SSQA, or ANSI/EIA-599). Identify what segment of the company (one division or the entire company) that received the award or certification. Describe when the award or certification was bestowed. If the award or certification is over three years old, present evidence that the qualifications still apply. To: (2)The offeror may submit a Contractor Performance Assessment Report (CPAR) in lieu of the past performance report prepared by Dun & Bradstreet.� The offeror may also describe any quality awards or certifications that indicate the offeror possesses a high-quality process for developing and producing the product or service required. Such awards or certifications include, for example, the Malcolm Baldridge Quality Award, other government quality awards, and private sector awards or certifications (e.g., the automobile industry's QS 9000, Sematech's SSQA, or ANSI/EIA-599). The questionnaire is due in accordance with section L.15. To: (3) Offerors may submit a Contractor Performance Assessment Report (CPAR) in lieu of Attachments S-2 and S-3. If a CPAR is not available, the offerors must submit a list of at least three references and send each listed reference the Client Authorization Letter provided in Part III - SECTION J as Attachment S-2 authorizing the reference to provide past performance information to the government.� Each listed reference must also be provided with the Past Performance Questionnaire provided as Attachment S-3 in Part III � SECTION J.� The questionnaire is to be completed by the reference and returned in accordance with the instructions contained thereupon.
