All information received in response to this notice is marked as proprietary and will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The Lebanon VAMC located at 1700 South Lincoln Avenue, Lebanon, PA 17042 is performing market research to determine if there is a sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small or emerging small business firms; or (4) large business firms to set aside a multiple award contract. Lebanon VAMC requires contractors to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program, in accordance with Veterans Health Administration (VHA) Handbook 1162.09. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. Contracted Emergency Residential Services (CERS) programs for Personal Care Home Services target and prioritize homeless Veterans transitioning from literal street homelessness, Veterans being discharged from institutions, including those in need of medical respite, and Veterans who recently became homeless and require safe and stable living arrangements while they seek permanent housing. CERS Programs, either directly or through linkage with community and other VA services, provide low demand time-limited services such as supporting mental health stabilization, Substance Use Disorder (SUD) treatment services, enhancement of independent living skills, vocational training, and employment services. Emphasis is placed on referral and placement in permanent housing or longer-term residential programs utilizing VA and/or community resources. Lengths of stay in CERS typically range from 30 to 90 days with the option to extend based on clinical need. See the attached PWS for a full description of required services. The anticipated period of performance is two (2) years, with task orders to be issued as services are required. All interested firms who can meet the requirement should respond, in writing, and provide the following: 1. Are you handicap accessible? 2. What is the Staff to patient ratio? 3. Do you have staff on site 24 hours? 4. Are you fully licensed as a personal care home? 5. Will you be able to transport Veterans to and from Lebanon VA Medical Center for medical appts? The NAICS Code 624221 is applicable to this acquisition, with a Business Size Standard of $11.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract.
