- ******************************AMENDMENT TO THE RESPONSE DATE********************************************************* RESPONSES ARE DUE MONDAY, OCTOBER 26, 2020 AT 12:00 PM (PST) ______________________________________________________________________________________________________ The Federal Emergency Management Agency (FEMA) has an urgent request and is considering the issuance of a firm fixed price purchase order for�ADA Portable Toilets and�ADA Handwashing Stations with services twice a week in support of disaster relief operations of DR4569 as a result of California Wildfires. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. FEMA is issuing a written solicitation in accordance with FAR Subparts 13.106-1 and Subpart 12.6. The solicitation number 70FBR921Q00000003 is being issued for a request for quotes (RFQ). The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 562991 Pumping (i.e., cleaning) portable toilets. The Service Contract Act of 1965, as amended (41 U.S.C. 351) is applicable to this procurement and the following wage determination shall be applicable to the resulting purchase order: Wage Determination No.: 2015-5673 (Rev.-10) This action is being competed using the policies and procedures for procurements under the Robert T. Stafford Disaster Relief and Emergency Assistance Act (42 USD 5150) and FAR 6.208 Set-Asides for Local Firms during a Major Disaster or Emergency. This action is a small business set-aside for vendors residing in or primarily doing business in federally declared disaster 4569�counties (Los Angeles, Madera, Mendocino, San Bernardino, San Diego, and Siskiyou)�in the state of California. Local Area Set-aside as set forth in provisions FAR 52.226-3, FAR 52.226-4 and 52.226-5.� Description of Requested Items/CLIN Structure CLIN 0001�(1) ADA Portable Restroom CLIN 0002�(1) ADA Handwashing Station CLIN 0003 (1) Portable Restroom CLIN 0004 Delivery/Pickup Fees Service Location at Willits Library 390 E. Commercial Street, Willits, CA 95490 for a period of performance of 30 days. Monthly amount shall include cleaning services twice a week, replenishing of supplies, and maintenance of each unit. CLIN 0005 (1) ADA Portable Restroom CLIN 0006��(1) ADA Handwashing Station CLIN 0007 (1) Portable Restroom CLIN 0008 Delivery/Pickup Fees Service Location at Ruth Lake Community Hall 591 Van Duzen Road, Mad River, CA 95526 for a period of performance of 30 days. Monthly amount shall include cleaning services twice a week, replenishing of supplies, and maintenance of each unit. OPTIONAL CLIN - Portable Restroom with Handwashing Station (This CLIN will be exercised when and/or an additional restrooms are required during the base�period of performance of 90 days. Quote Submission Instructions Responses to this Request for Quote are due no later than 5:00 pm PST, on Friday, October 23, 2020 and should be submitted electronically to ashlee.young@fema.dhs.gov, Demetria.carter@fema.dhs.gov, and tiffany.young@fema.dhs.gov. This solicitation requires an active registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. All quote submissions must include the following information on a quote submission cover letter. Delivery Schedule The contractor shall propose availability, date and time for twice weekly service/cleaning at Willits Library 390 E. Commercial Street, Willits, CA 95490 and Ruth Lake Community Hall 591 Van Duzen Road Mad River, CA 95526. � The contractor shall also confirm whether they are able to cover all declared disaster counties as indicated above for DR4569.�Any additional service location will be requested within a 24-48 service window after�request. � Factor 2. Price The contractor shall complete the attachment -�Pricing Chart�a pricing, delivery, and service schedule which includes the necessary maintenance, supplies and service to each restroom and handwashing station. � The contractor shall provide images of portable restrooms and handwashing stations. � Factor 3. Local Area Set Aside as set forth in provisions FAR 52.226-3, 52.226-4, and 52.226-5 � Factor 4: Past Performance The contractor shall provide 3 references which demonstrates work similar in size, scope, and complexity to this requirement������� ____________________________________________________________________________________ � Additional Provisions and Clauses i.� 52.252-1 Solicitation Provisions Incorporated by Reference. ii.� The provision at 52.212-1, Instructions to Offerors-Commercial, is incorporated by reference.� iii.� The provision at 52.212-2, Evaluation-Commercial Items, is applicable in that the Government will make awards resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government based on price only.� iv.� Offerors are to complete the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.� v.� The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and is incorporated by full text.� However, separate invoice instructions with payment details will be provided at time of award of an IDIQ.� See the attached solicitation vi.� The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
