STATEMENT OF WORK INTRODUCTION AND SCOPE OF WORK This contract is to provide support services on the Automed equipment as identified below, located at the Department of Veterans Affairs Medical Center, West Palm Beach, FL. Services shall be performed by manufacturer trained service engineers. Coverage shall begin on 04/15/21 and end on 04/14/22 with Four (4) consecutive One-Year Options at the discretion of the Government. Support shall be provided via telephone. All equipment shall be maintained in proper operating condition as specified by the manufacturer. Equipment: Automed Automated Prescription Dispenser VA Inventory ID # 45511 S/N B00083 Model: OPTIFILL2 Location: 1B111 REQUIREMENTS Contractor shall provide support services in accordance with manufacturer s specifications and as outlined below: Provide all labor, materials, parts, and travel to support the Optifill2. Provide unlimited telephone/modem support for software and hardware. Provide telephone support Monday-Friday, 8AM EST-5PM EST. Includes all software upgrades. Response time: 1 hour by telephone, Monday through Friday from 8:00 am to 5:00 pm EST and 4-hour on-site emergency service response after completion of telephone/modem support. Contractor shall have twenty-four (24) hour access to all needed parts. 7) Contractor shall provide evidence of factory training for each service engineer upon request. V. PROGRESS AND COMPLIANCE Vendor shall provide evidence of factory training for each service technician upon request. B. Contractor shall provide certification worksheets on each machine tested. C. Contractor shall contact Biomedical Instrumentation, (561) 422-TBD to arrange for service. D. Service reports shall be provided to Biomedical Engineering within 24 hours of completing service. The report shall include complete details of all work performed. E. The contractor shall provide certification in the form of technical documentation, original invoice, or service documentation that the quality of updates and upgrades is sufficient to guarantee continued performance and operation of the system listed in this contract.
