- SOURCES SOUGHT NOTICE This is a SOURCES SOUGHT NOTICE only. This is not a request for formal proposals or quotes. No formal solicitation document exists at this time. This request is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this notice will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. PURPOSE AND OBJECTIVES: The intent of this notice is to identify capable sources interested in providing LAB SPECIMEN COLLECTION AND TRANSPORT SERVICES for the Buffalo VA Medical Center located at 3495 Bailey Ave, Buffalo, NY 14215. The anticipated contract action is expected to be a Fixed-Price, IDIQ Contract with a five-year ordering period, tentatively from 01-01-2021 to 12-31-2025 All responsible sources who believe they can furnish the service described in this notice may submit a capability statement clearly establishing an ability to provide the required service in the place of performance by emailing Joseph.Cellino@va.gov with details regarding your firm s capability no later than 4:00PM EST on November 24, 2020. Additionally, responses must include firm s location and area of performance as deliveries of this nature are time sensitive, with STAT labs requiring service within 1 hour of request. BRIEF DESCRIPTION OF REQUIREMENT: The contractor must provide the pick-up and delivery of blood specimens and blood products between the VA Western New York Healthcare System and local hospitals and Reference Labs within 1.5 hours from the time the request for services is generated, 24 hours/day; 7 days/week, 365 days a year, including holidays. The contractor must have the ability to supply an occasional STAT delivery request entailing delivery times of less than 1 hour from time of request. Contractor must supply service to destinations including, but not limited to, the following Pick up/Delivery Addresses AMERICAN RED CROSS REFERENCE LAB 825 JOHN ST WEST HENRIETTA, NY 14586 IMMCO 10 EARHART DRIVE, SUITE 100 WILLIAMSVILLE, NY 14221 BUFFALO GENERAL HOSPITAL 100 HIGH ST BUFFALO, NY 14203 ERIE COUNTY MEDICAL CENTER 462 GRIDER ST BUFFALO, NY 14215 KALEIDA HEALTH -CENTER FOR LAB MEDCINE 115 FLINT RD AMHERST, NY SISTERS OF CHARITY HOSPITAL 2157 MAIN ST BUFFALO, NY 14214 OISHEI CHILDREN S HOSPITAL 818 ELLICOTT STREET BUFFALO, NY 14203 SOUTH BUFFALO MERCY HOSPITAL 565 ABBOTT ROAD BUFFALO, NY 14220 VA WESTERN NY HEALTHCARE 3495 BAILEY AVE BUFFALO, NY 14215 QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The contractor has to be Department Of Transportation (DOT) certified. The Contractor must have documented Blood Borne Pathogen Training for its employees. ESTIMATED QUANTITIES: The Buffalo VA estimates, but does not guarantee, that approximately 40 trips per month will be required for the duration of this contract. Payment will be on a per delivery basis.
