Description
Water Treatment Consultation Services for the New Jersey Health Care System, East Orange Campus and Lyons Campus Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-02. Updated through FAC 2021-02 effective 11/23/20; and Acquisition Policy Flash 21-06 effective 11/16/20. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB). Consultation at both East Orange and the Lyons campuses VA New Jersey Health Care System. Scope of Work: East Orange Campus: Provide all labor, personnel, supervision, tools, and all materials necessary to provide consultation services as defined below. Lyons Campus: Provide all labor, personnel, supervision, tools, and all materials necessary to consultation services as defined below. Contractor is to meet with chemical Contractor and on-site personnel on a monthly basis. Review all water treatment reports and correspondence as they are submitted. Prepare program logs. Prepare a monthly treatment program review. Perform additional chemical testing as needed. Inspect water treatment equipment. Assist with the annual review and goal setting. Discuss water related problems and questions as needed and make recommendations to correct deficiencies. Background: VA NJ Health Care System requires Water Treatment Consultation services at the East Orange and Lyons Campuses. The contractor shall propose a timely schedule to meet the facilities needs and requirements. All work is to be performed between 7:00 AM to 3:30 PM, Monday thru Friday excluding Federal Holidays. Lyons, NJ 07939 CHANGES TO STATEMENT OF WORK (SOW): Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer (CO). REPORTING REQUIREMENTS: Reports: Service reports must be signed by both service provider and COR at completion of work. CONTRACTOR EXPERIENCE REQUIREMENTS: Contractor must have a minimum of three (3) years working with all equipment listed above and a minimum of three (3) years in Water Treatment Consultation services. Service person must retain the OSHA 30 Hour Safety Training Card. Award shall be made to the contractor whose quotation offers the best value to the government considering: 1) Technical Capability; (2) Price; and (3) Past Performance. Technical capability - Contractor shall provide a maximum 10-page capability statement demonstrating their ability to meet all of the requirements of the SOW. Technical Proficiency: Contractor shall submit a proposal that demonstrates its understanding of the requirement in accordance with the Statement of Work and the extent to which potential risks are identified and mitigated. Contractor documentation must demonstrate that they are a business regularly engaged in Water Treatment Consultation and Water Treatment Service. Technical Experience: Contractor shall supply documentation on 3 5 contracts in the past 5 years where they performed work of similar size and complexity. Price: The Government will evaluate the price by adding the total of all line item prices, including all options. The Total Evaluated Price will be that sum. Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). Past Performance Analysis: The offeror shall provide 3-5 instances of past experience of similar scope within the past 5 years. The contractor shall be a firm regularly engaged in Water Treatment Consulting services. Individuals with no past performance shall receive a neutral rating for this evaluation factor. Past Performance Questionnaire (attached) to be completed and submitted on offeror s behalf that speaks to offeror s performance on active and physically completed contracts similar in nature to this requirement in accordance with the Statement of Work.
