- PERFORMANCE WORK STATEMENT Femtotrain and Insight Laser Maintenance Contract 523-21-1-8176- OVERVIEW: The vendor, Spectra-Physics, shall provide all personnel, labor, equipment, tools, materials, supervision and other items and services necessary to perform maintenance on our Femptotrain and Insight DS+ lasers. These lasers periodically require repairs and there is no one at the Jamaica Plain VA or Boston VA in general with the requisite knowledge of the lasers to perform this work. The lasers are highly specialized instrument, purchased originally from Spectra-Physics, and there are no third parties that can provide the required service. This contract is for 3 years and covers the GOLD level service for the laser, as described in the attached quote. The 3-year contract offers a significant discount over purchasing the contract year to year. It will be extended to 5-years, which allows the approved discount, pending on availability of funds. 1. EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited to, all Personal Protective Equipment (PPE). The two lasers that will be covered, Insight DS+, s/n 1361 and Femto-Train 1040-5, s/n 10801-0108, are highly specialized instruments and there are no third parties that can provide the required service. This contract is for three years. The three-year contract offers a significant discount over purchasing the contract year to year. 2. AREAS TO BE SERVICED: VA Boston Healthcare System 150 South Huntington Ave Boston, MA 02130 3. TASK FREQUENCY AND INSTRUCTIONS: This will be an annual service for three years, at the VA Boston, Jamaica Plain campus. This will be sufficient to complete the work set forth in the Statement of Work. This visit is to be scheduled in advance with the POC: Rebecca Mathias at 857-364-5604. 4. SPECIAL WORK REQUIREMENTS: Contractors shall be accompanied by VA personnel at all times. PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. 6. ATTACHMENTS: Quote from Spectra-Physics. 6500.6 VA Security Checklist 7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the con tractor under any conditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Possesesion of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court. 8. COMPLIANCE WITH OSHA AND ANSI LASER SAFETY STANDARDS: The contractor shall comply with the Federal OSHA and ANSI Laser Safety Standards. The contractor shall: Have methods by which all employees are educated as to risks associated with laser safety. Have policies and procedures which reduce the risk of employee exposure to lasers. Have mechanisms for employee counseling and treatment following exposure to lasers and laser generated airborne contaminants. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract.
