Describe in detail the need for the changed work, time, or cost. Modification required to replace no hub coupling iron pipe with 3 L type copper pipe and pro press fittings with crimped copper couplings due to system pressure differential caused by new SPS sump pumps. Amount of within scope vendor effort requires additional time and cost to complete during the ongoing COVID-19 pandemic. 2. A statement addressing whether the proposed modification is within the original scope of the contract and specifically addressing the facts considered in reaching the conclusion. Modification is within scope as new SPS sump pumps pressure differential require replacement of 140 lf of no hub coupling iron pipe with 140lf of 3 L type copper pipe and pro press fittings with crimped copper couplings due to system pressure differential, seismic bracing, and 3 check valve. Facts and alternatives in reaching this conclusion is: 1) modify the contract, 2) solicit required modification action under a separate procurement, 3) fulfill current contract and have an inoperable system, or 4) terminate contract for convenience. The best and only alternative is to modify the existing contract due to time constraints and warranties. Requested modification action is within scope of base contract and will permit continued performance without severely impacting the hospital mission of patient care. 3. A statement analyzing the pricing factors considered in reaching a fair and reasonable price determination and identifying each Line Item Number affected, including but not limited to, IGCE, market research, and any other price analysis information. Pricing factor considered in reaching a fair and reasonable price determination include technical review by station Subject Matter Expert (SME), vendor quote, and contracting officer personal experience. 4. Were technical and legal reviews conducted based on VHAPM Part 801.602-70 Contract Reviews SOP, if not, attach HCA approved exemption, if applicable. Technical review required one level above CO per contract matrix due to time (exceeds 60 calendar days). Legal review not required since modification does not exceed $100K.
