- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing digitizing records. These are for the Coatesville VA Medical Center in 1400 Blackhorse Hill Road, Coatesville, PA 19320. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of services listed in the attached document. Responses should include the following information: Verify that the business is under 541519 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point-of-Contact, if in possession of a Federal Supply Schedule contract, and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 P.M. (EST) on Tuesday, May 18, 2021. No submissions will be accepted after this date and time. Questions can be submitted electronically to Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. STATEMENT OF WORK 1. Contract Title: Installation of Additional ResidentGuard in Geriatric and Extended Care Units 2. Background: The Coatesville VA Medical Center (VAMC) has Geriatric and Extended Care (GEC) units that have mobile patients. The Coatesville VAMC would like to ensure the safety of these patients by providing a system that alerts nursing staff of patients trying to leave the unit. The Coatesville VAMC currently has the ES2200 ResidentGuard system by Accutech on several of the GEC units. Expansion of the system is requested to incorporate additional doors. 3. Scope: Contractor is to provide, install, and verify proper operation of the ResidentGuard system on the designated GEC units. 4. Specific Tasks: The contractor shall provide, install, and verify proper operation of the following equipment: Building 138E Hospice Unit (one (1) single doors) One (1) 4 Internal Single Door Kit including door controller, TX wand, keypad, cable kit & door contact One (1) 6 -8 Internal Double Door Kit including door controller, TX wand (2), keypad, cable kit & door contact (2) One (1) KP103 Keypad Two (2) Local Alarm w/Back Box One (1) KP403 Outdoor Keypad Two (2) Magnetic Lock, 3101 Series, NFPA 101 Delayed Egress Two (2) Receiver Mount, Short Lexan 22/12, Non-Shielded Cable Building 138A Patio Entrance (one (1) double door) One (1) 6 -8 Internal Double Door Kit including door controller, TX wand (2), keypad, cable kit & door contact (2) One (1) Local Alarm w/Back Box One (1) KP103 Keypad One (1) Panduit Double Door Kit Two (2) Receiver Mount, Short Lexan 22/12, Non-Shielded Cable The respective equipment listed for 138A must communicate with the current ES2200 ResidentGuard systems installed on 138B. The respective equipment listed for 138E must communicate with the current ES2200 ResidentGuard systems installed on 138E. The contractor shall set up and install each device as needed in order to communicate with the ResidentGuard system. The equipment shall function using the current Accutech sensors worn by the patients. The contractor shall acquire a penetration permit from the Coatesville VA Fire Department when penetrations are required. All penetrations made by the contractor must be sealed with Hilti fire caulk before moving on to the next location. The contractor shall provide a standard one-year warranty on all equipment listed above. 5. Performance Monitoring: The contractor shall keep the COR informed of work complete each day. The COR shall randomly inspect the work in progress. 6. Security Requirements: All contractor representatives who perform this work shall acquire the appropriate VAHCS identification badges. No other special security requirements are anticipated. Biomedical Engineering staff must be present for any work performed within Information Technology Communication closets. Contractor shall be working in an area where patient care takes place with potential for incidental exposure to patient information. Contractor shall not disclose any patient information. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI): There is no GFE or GFI anticipated with this contract. 8. Other Pertinent Information or Special Considerations: a. Identification of Possible Follow-on Work. There is no follow-on work anticipated. b. Identification of Potential Conflicts of Interest (COI). None identified. c. Identification of Non-Disclosure Requirements. Contractor shall be working in an area where patient care takes place with potential for incidental exposure to patient information. Contractor shall not disclose any patient information. d. Packaging, Packing and Shipping Instructions. There are no special requirements. e. Inspection and Acceptance Criteria. Functionality of the installed ResidentGuard system will be verified by Biomedical Engineering. 9. Risk Control: Contractor is responsible for providing all applicable training and protective equipment to protect Contractor s employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 10. Place of Performance: All work will be performed at the VA Medical Center, 1400 Blackhorse Hill Road, Coatesville, PA, 19320. 11. Period of Performance: Contractor will have 120 days from contract award to complete the installation. 12. Delivery Schedule: Contractor will set delivery schedule to ensure that installation can be completed within 120 days from contract award.
