- Request for Information (Sources Sought) 36C25921Q0535 Page 4 of 4 TV Services Montana VA Health Care System Fort Harrison, MT 59636 BRIEF SCOPE OF WORK (This is for information only) The Montana VA Health Care System requires a contractor to provide tv services, in accordance with the terms and conditions stated within this abbreviated Performance Work Statement (PWS). This is a Request for Information (RFI) Only for Market Research purposes, no Award will be made from this RFI. TASKS and REQUIREMENTS: The Contractor must provide satellite/cable tv service that will operate as a stand-alone system with no cross over to the medical center systems for buildings 154 & 154a, Behavioral Health Unit (BHU) and Liberty House. If cable tv is provided, the Contractor is required to provide 70 Digital Transport Adapters (DTAs) otherwise known as cable boxes, these are to include a remote control for each box. The Contractor must ensure that there will be no opportunity for any breach of confidential information or access to medical records from any wireless network system. The Contractor must provide each patient with a remote control and a help line number for technical support. The VA Office of Information & Technology (OI&T) will sign a Memorandum of Understanding with the Vendor to allow access to areas where wireless equipment is located for technical trouble shooting, replacement, and/or repair of equipment. The remote controls will be left with each TV and/or cable box or Digital Transport Adapter (DTA) and available to the users via access to locations with TVs and/or by designated VA Medical Center personnel within the applicable services. The satellite/cable tv service provided will include a variety of local channels, sports channels, news channels and family viewing channels. The Contractor must conduct a functionality survey before proceeding with any installation. The contactor guarantees all equipment covered in this contract shall be in optimum working condition. All changes updates or retrofits made on any component or system shall be annotated on station equipment manual and records. Services are to also include, recording all routine work, corrections, and repair work in the equipment log. The Montana Veteran Affairs Health Care System will not furnish part and/or test equipment for the performance of this contract. It is the responsibility of the contractor to provide the appropriate equipment and/or supplies necessary to complete the work as required within. The Contractor must maintain and troubleshoot all issues regarding the equipment associated with the cable tv service. The vendor will maintain and troubleshoot additional cable tv services if there is an expansion of services. The Contractor must repair equipment as needed to ensure the satellite/cable tv service is operable 24/7 for patient and patient family use. Current equipment cannot be removed unless it is replaced with new equipment that has the same or greater performance qualities of service for patients. The main WIFI Router for connection of all DTA (cable box) units will be located in the main communications closet (B1A). The hardware will not be connected to the VA Network or other Secure Networks. 24/7 technical support by Vendor to the user is required. A phone number must be provided to customers/users. All technical support must be provided by the vendor. Contractor must provide installation of all hardware and cabling and install additional equipment, if replacement is needed, to provide current level of cable tv service, approval must be obtained for any repair must be approved by the contracting officer. If replacement is needed, the hardware (including cable, junction connections, and other equipment/items) will be the property of FHM VAMC and will remain onsite at the end of the contract. System for password protection of wireless network will be provided by the vendor. The satellite/cable tv service will utilize the wifi service that is separate from the hospital network to alleviate any security issues. If outages occur the Contractor will inform Contracting Officer Representative (COR) of action plan to fix/repair network within 24 hours if wireless network is not made operable within 4 hours of incident (during normal working hours) or 12 hours of incident (during non-working hours). The repair of the network must be completed within 3 days after the action plan is submitted. Any day after 3 days must be an agreement between the COR and the Vendor. The Vendor will credit the government/agency credit for each day the network is down after 3 days. Contractor must provide training on use of system to COR & POC. No training will be provided to patients or visitors. Contractor must provide contact information to user for technical support (e.g. business card; posters in patient rooms). SCHEDULING ARRANGEMENTS: Except by special alternative arrangement, scheduled work will be performed only during VAMC s normal working hours (8:00 a.m. to 4:30 p.m. Monday through Friday excluding federal holidays). Vendor shall provide at least two weeks notice to the Contracting Officer s Representative (COR) prior to performing the work. Only two ice makers at a time can be out of service for the inspection, service and cleaning procedures. Work must be conducted when the COR or his/her designate is available on site to monitor the work. Check in/out procedures: Prior to commencement of and immediately after each service visit, the contractor and/or contractor s authorized repair technician must check in and out with the VAMC COR. FEDERAL HOLIDAYS: Federal holidays are any calendar days designated by statute or executive order as federal holidays. Federal holidays consist of New Year s Day, Martin Luther King Jr. s Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas. DOCUMENTATION: Contractor shall provide this VA HCS with individual service reports, which describes the inspection services performed on each piece of equipment. Provide complete written report of the inspection fully describing all services performed and certification that unit has been cleaned and sanitized. The report shall contain copies of preventive maintenance procedures used along with any logs taken. Furnish report within two weeks of each facility inspection by email to the COR. All safety-related deficiencies shall be immediately reported to the COR, during the inspection visit. PROTECTION FROM DAMAGE: Contractor shall provide all labor, materials, and equipment necessary for protection of VA HCS personnel, furnishings, equipment and buildings from damage, and shall replace or repair any damage due to work performed under this contract equal to its original construction and finish. RESPONSE REQUESTS: Contractor shall only respond to requests for service from VA HCS CO or COR. If service is requested from any other source the Contractor is to advise the caller of this requirement and refuse to respond. Any billing resulting from unauthorized service will not be paid by the government. CHECK IN/OUT PROCEDURES: Prior to commencement of, and immediately after each service visit, the Contractor or his authorized repair technician must check in and out with the VA HCS COR (during normal working hours). Contractor will be required to obtain contractor badges provided by the VA HCS. LIABILITY INSURANCE: Contractor must provide this VA HCS with written confirmation that Contractor has adequate insurance protection against negligent work and other forms of liability. The nature and level of protection must be acceptable to this VA HCS. INFORMATION SECURITY: The C&A requirements do not apply, and a Security Accreditation Package is not required.
