- PRODUCT DESCRIPTION VA PITTSBURGH BETA TEST SITE for DSS INC DIASYST SOFTWARE Purchase of DSS Diasyst Intervention Recommendation Engine - Type 2 Diabetes and Hypertension Annual Limited License and Patient Mobile Application as a Beta test site for Base plus 1 OPY Includes Bug fixes, Minor Release Upgrades, and Telephone Support for one year, Level 1 facility for Base plus 1 OPY DSS Diasyst Implementation to include set-up, training and go-live for Base year only VA Pittsburgh will be supplying own VM builds & hosting on Premises via COTS The mobile app is not approved for use by VA. The veterans are allowed to upload their own data to the app since they own their data. The VA clinicians will be able to download/view the data via the web portal but not on the mobile app. Please see attached MOU describing the responsibilities of both DSS and VA Pittsburgh as it relates to product testing. Product Overview Diasyst is a software-based diabetes management platform provides a comprehensive algorithm driven approach that enables the healthcare providers timely receive the CBG data and safely deliver virtual diabetes management while maintaining self-care engagement via features such as interactive glucose monitoring mediated education and medication reminders. Using a mobile app, patients can transmit their CBG securely and seamlessly to their healthcare providers. The VistA integrated, web-based portal that aggregates the pertinent clinical information and based on preset mechanisms, alerts providers such as PACT nurses, HT coordinators, diabetes educators, and clinical pharmacists. Besides alert mechanisms and clinical dashboard, the portal has in-built guidelines based algorithmic recommendations for further therapeutic intervention. These recommendations transfer to patient s medical records thus streamlining workflows for the provider while ensuring safety in communications with patients. Diasyst creates a feedback loop that improves access to care, connects providers and patient in real-time thus offering technology that empowers the clinical teams by improving efficiency while maximizing patient safety, and presumably, the quality of care
