- VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This solicitation is issued as a request of quotations(RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. This solicitation is set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1000 employees. The Department of Veterans Affairs, Aleda E. Lutz VA Medical Center, Saginaw, MI is seeking to purchase six Capsa Carelink M48 Mobile Computing Carts for their Bad Axe, MI Community Based Outreach Center (CBOC) expansion. Please see customer s Statement of Work below for details/salient characteristics. This is a BRAND NAME requirement. STATEMENT OF WORK Department of Veterans Affairs Aleda E. Lutz VAMC Statement of Work for Mobile Computer Cart Solution 1. PURPOSE The Aleda E. Lutz VAMC has a requirement for six Capsa Carelink (M48) mobile computer carts for FY 21. The facility is scheduled to take occupation of the Bad Axe CBOC expansion in 2021. These mobile computer carts are needed for this expansion. 2. SCOPE M48 Medical Cart Electrical lift Carelink dual swivel monitor mount 7"" glass centralized communication and cart adjustment panel with cart management software N-Stride steer assist Lithium battery worksurface Keyboard Floor lighting package Adjustable keyboard with left/right mouse holder 5"" casters with dual wheel locking Power cord Lithium battery Three year warranty required 3. DELIVERY COORDINATION: Normal business hours for acceptance of deliveries are 8:00am 3:30, Monday through Friday excluding Federal Holidays. Deliveries must be coordinated in advance with the Chief of Logistics, Jason Christianson (Jason.Christianson@va.gov). All items included in the requirement shall be shipped and delivered to site at the same time. If deliveries must be broken into several shipments due to size, this must be coordinated in advance with the Chief of Logistics. Failure to coordinate the shipment with the Chief of Logistics may result in failure to deliver and cause re-scheduling of delivery/freight. Address: Aleda E. This is an open-market combined synopsis/solicitation for products as defined herein. The government intends to award a purchase order as a result of this combined synopsis / solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
