Description
(i) This is a Combined Synopsis/Solicitation for Brand Name Midmark Patient Support Rails Top as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price. The solicitation number is 36C2442Q1183 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-05 dated 03/10/2021. This solicitation is set aside 100% Total Small Business and the associated NAICS code 339113 has a small business size standard of 1,000 employees. Schedule of Requirement/CLINS Statement of Need and Salient Characteristics This requirement is BRAND NAME MIDMARK PATIENT SUPPORT RAILS STATEMENT OF WORK Scope of Work: VA Butler Healthcare System (529) has a BRAND NAME ONLY requirement for twenty-two (22) Midmark Patient Support Rails for the Midmark Exam Tables in Primary Care. Background: Midmark Patient Support Rails are needed for the Midmark Exam Tables located in the Primary Care Department. Patient Support Rails provide patients a gripping surface for entering, exiting, or repositioning on the exam tables. Salient Features: Midmark Patient Support Rails - Model 9A561001 Rotation/Locking Points Patient Support Rails can be locked into three positions front horizontal or two elevated positions- or moved to a fully back horizontal position. Visual indicator marks help guide the operator to the different locking positions. On-Chair Positioning When rotated to the back horizontal position, the support rails, designed to improve patient security and provide a gripping point during movement into supine, prone, left or right lateral positions. Continuous Gripping Surface Patient Support Rails Provides patients with a 1ΒΌ inch diameter continuous gripping surface for entering, exiting, or repositioning on the exam table. They can be used as an affixed gripping point for patients to independently raise themselves off the chair. Specifications: Brand name requirement: 9A561001 Midmark Patient Support Rails, Field Installed; 22 each Training and Installation not required The Certification & Accreditation (C&A) requirements of VHA Handbook 6500.6 do not apply to this requirement: therefore, a Security Accreditation package is not required. No C&A or MOU/ISA is required. Sensitive information exposure as part of this contract involves applicable security controls within the facility as part of the VA Information Security Program. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
