- VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05 3-10-2021 This solicitation is set-aside for Service-Disable Veteran Owned Small Business The associated North American Industrial Classification System (NAICS) code for this procurement is 336112, with a small business size standard of 1000 Employees The FSC/PSC is 6515. The Michael E. DeBakey VA Medical Center (MEDVAMC) is requesting to purchase five (5) replacement Haag-Streit Beam splitters 50/50 for BQ900 Slit Lamp and 5 replacement Haag-Streit Short Observer Tube w/eyepiece for BQ 900 Slit Lamp for the Eye Clinic. See attach Statement of Work (SOW) All interested companies shall provide quotations for the following: Supplies/Services: STATEMENT OF WORK OPHTHALMIC EXAM LANE(S) UNIT Purpose The Michael E. DeBakey VA Medical Center (MEDVAMC) is requesting to purchase five (5) replacement Haag-Streit Beam splitters 50/50 for BQ900 Slit Lamp and 5 replacement Haag-Streit Short Observer Tube w/eyepiece for BQ 900 Slit Lamp for the Eye Clinic. The Haag-Streit Beam Slitters 50/50 and Haag-Streit Short Observer Tube w/eyepiece are two attachments that are utilized with all funduscopic eye exams for diagnostic purposes. Scope Contractor shall supply, install, and provide all required electrical interfacing so all ophthalmic equipment operates as one unit. The contractor shall provide training to all staff members (and staff support members) who will be utilizing these ophthalmic exam lanes at no additional cost to the U.S. government. Required Equipment All products must meet all salient characteristics defined in this section. If other than brand name (Or Equal/Equivalent/Substitute Products) are to be provided then the contractor shall furnish catalog, part number, and the characteristics of the substitute item that are equivalent to the brand name. Other documents may also be required in order to clearly describe the item(s) so that it/they can be evaluated by the end user. (Items otherwise equal may not satisfy the requirement due to differences in material composition, size, environmental application, etc). Salient Characteristics The beam splitter can permanently be left on the slit lamp for diagnostic purposes giving full 100% light to the oculars. For use with video or second observer tube there is a choice of either 50/50% or 70/30% the latter giving more light to the camera, thus an even better image. The Haag Streit Short Observer Tube is the second observer tube monocular and takes place in the right or left-hand side of the beam path of the stereomicroscope depending on the slit lamp connection. Item # Description/Part Number* Qty 0001 Haag-Streit Beam Splitter 50/50 for BQ 900 Slit Lamp (09007.10.50/50) 5 0002 Haag-Streit Short Observer Tube with Eyepiece for BQ 900 Slit Lamp (09007.7) 5 System Requirements Contractor shall provide a projected schedule in terms of days from the time of award addressing (at a minimum) the following milestones: verification that all system quantities and configurations are accurate; manufacture/assembly of all parts and/or equipment; delivery to identified location; and complete installation of hardware. Training Contractor shall provide comprehensive training on the use of the equipment for the operating clinicians. This training program shall include at the minimum the following: (i) Technical Training, (ii) Operational Training, and (iii) Hardware and Software Troubleshooting. Training shall occur onsite during normal business hours, 8:00am-4:30pm, Monday through Friday. Competency assessments shall be provided with the training. Contractor shall provide technical training for two biomedical engineering personnel. The cost for this training and all training related expenses such as travel, lodging, and other expenses are included in the contract price. This training shall cover the preventative maintenance, troubleshooting, and general maintenance of the equipment. Training shall allow for classroom instruction as well as individual hands on programming, troubleshooting and diagnostics exercises. Competency assessments shall be provided with the training. TESTING AND CERTIFICATION Upon installation of each piece of equipment in the system, the Contractor shall notify the COR in writing via email that the system is ready for acceptance testing, and that it meets all requirements as specified. Submission of this notification of system readiness shall be accomplished by the contractor prior to the beginning of the scheduled Government Acceptance Test. GOVERNMENT ACCEPTANCE TEST Contractor shall perform the Government Acceptance Test in the presence of the Contracting Officer s Representative (COR) and designated Ophthalmology Staff for proof-of-performance. Upon completing the Government Acceptance Test successfully, Contractor shall document the success of this Government Acceptance Test on Contractor s company form which will be certified by the COR s signature as an evidence of Government Acceptance of the referenced piece of equipment. A copy of this document shall be furnished to the COR and the Contracting Officer before Contractor s Representative at the end of each testing. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Lowest Price Technically Acceptable (LPTA) The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" Submission of your response shall be received not later than September 10, 2021 at Ayanna.Hamilton@va.gov hand delivery will not be accepted. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
