Description
This is a SOURCES SOUGHT NOTICE/REQUEST FOR INFORMATION ONLY. Responses to this notice will be used for information and planning purposes and this notice is being posted to both GSA e-BUY and to Contract Opportunities at (https://www.beta.sam.gov). No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to sources sought notice is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Department of Veterans Affairs Medical Center, Veterans Health Care System of the Ozarks (VHSO), Fayetteville, Arkansas has a requirement for onsite Mobile Laser/Lithotripsy equipment and related technical personnel and support services to eligible beneficiaries. The VHSO anticipates an average of ten (10) veterans will be treated per month. The per-procedure pricing shall include single stone and multiple stone procedures per unilateral or bilateral episode. The equipment must be prepared to perform the first procedure of the day at 8:00 a.m. and continue to perform procedures until the VHSO s schedule for the day is completed. Contractor equipment must fit through elevator doors of 4-foot-wide and 7 foot tall. Contractor s technical personnel shall be certified and trained in lithotripsy and laser services and operation of the equipment. The Technician(s) will assist the attending Urologist and other physicians in the operation of the equipment. The technician may be required to operate the equipment and any related imaging equipment during procedures. Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted license to practice the individual profession. The qualifications of such personnel shall be subject to review by the VHSO Chief of Staff and approval by the VHSO Facility Director. Contractor personnel providing contract services shall be licensed as a Radiologic Technologist. Contractor payment for services may be withheld if service is provided or authorized by unlicensed personnel, without regard to whether such services were medically necessary and appropriate. The VHSO, Fayetteville, Arkansas shall not approve of subcontracts with unlicensed individual or group providers. Place of Performance: Contractor shall furnish services at Veterans Health Care Systems of the Ozarks, 1100 N. College Ave., Fayetteville, AR 72703. Offeror s response to this Notice shall be limited to 10 pages and shall include the following information: Offeror s company name, DUNS, Federal Tax ID, address, point of contact, title, phone number, and email address Offeror s interest in bidding on the solicitation when it is issued whether it be issued to VA FSS/GSA Schedule holders via e-BUY or a set-aside on SAM Contract Opportunities (https://www.sam.gov). Please provide VA GSA schedule number if you have one that covers onsite Lithotripsy Laser service in Fayetteville, AR. Offeror s type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hub-zone, 8(a), Woman Owned, Small Disadvantaged, etc. The NAICS code is 621999, All Other Miscellaneous Ambulatory Health Care Services (size standard of $7.5 million). Responses must be received by no later than 11:00am Central Daylight Time, 22 September 2021. After review of the responses to sources sought notice, a solicitation announcement may be published on the SAM Contract Opportunities website or to GSA e-BUY. Responses to sources sought notice are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to sources sought notice. All interested parties are reminded to be registered with System Award Management (SAM) at https://beta.sam.gov/ in order to be eligible for award of Government contracts.
