This was competed full and open and below are the estimated amounts for the periods of performance: Period of Performance 1 (7/1/2020 06/30/2021): $4,520,464.92 Period of Performance 2: $4,523,002.80 Period of Performance 3: $4,103,393.04 Period of Performance 4: $4,142,121.96 Period of Performance 5 (ending 06/30/2025): $4,192,591.68 Grand Estimated Total of 5-year IDIQ: $21,481,574.40 This contract will upgrade from a PACT 3 to a PACT 4 model, after completion of build-outs. CBOC provides both Primary Care and Mental Health services.
