Description
Synopsis: Network Contracting Office (NCO) 2 of the Department of Veterans Affairs intends to award a sole source contract to Stericycle, Inc located at 28161 N Keith DR, Lake Forest IL 60045, under the authority of 41 U.S.C 253 (c) (1) FAR 6.302-1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. The proposed procurement is a Control Substance Waste Disposable. Stericycle, Inc is the Original Equipment Manufacturer of this service. This will be a base period with four (4) one-year option renewals. The following items will be delivered as needed: Scope of Work: This process must render the controlled substance secure and unusable. The control substance disposal program (controlled substance wastage only) is for the safe and secure destruction of controlled substances wasted throughout the hospital. The program includes a secure container with activated carbon that deactivates the controlled substance and a deterrent (Bittrex) to prevent ingestion of the contents. A professionally trained technician will observe the containers on a monthly basis and monitor fill levels. If needed the containers will be replaced. On scheduled service days, the service technician will identify the containers that need to be exchanged and write the locations on a witness verification form. The GEMS Program Manager or Safety authorized employee will witness the technician adding water to the bottom of the fill line. Customer will sign the witness verification form document outlining how many containers required service, where they are located and verify that water has been loaded to activate denaturant. The technician will proceed to the locations needing container replacement and add a solidifier to the full container that is being exchanged before removing from the bracket. The full container will then be swapped out for the unused container and the bracket will be locked. After the service is complete, the technician will bring the full container(s) to the pre-designated area that contains the prepaid shipping boxes. Each container will be packaged and each box labeled with the preprinted shipping address label. The technician will prepare the full, solidified containers for shipment and disposal. All deactivated controlled substance waste will be shipped to a permitted medical waste Incinerator for treatment and disposal. At the end of the service the technician will hand off the witness verification form to the GEMS Program Manager. The Contractor will provide the customer with a report showing the number of containers purchased and disposed. The Contractor will automatically ship a replacement container for each received for disposal. The Contractor will provide a service log consisting of service locations and fill levels to be kept in the hazardous waste accumulation room in GC-34. The Contractor will provide the necessary training to James J. Peters, VHA staff during implementation of the process. Main Objectives: Containers must meet DEA regulation for deactivation (charcoal) and deterrent (Bittrex) of controlled substances this is not regulated medical waste. Acts immediately to deactivate controlled substance waste in the container (charcoal) Deterrent to prevent ingestions of the liquid while in use (Bittrex) Solidification of waste prior to shipping secures content and prevents diversion One-way disposal path- no way to remove contents, the mouth of the jar is soldered Designed to prevent sharps, vials or ampules from entering the container, designed in a way that prevents removal or modification of this opening Lockable wall mount brackets Provide a kit to ship for treatment (incineration) included in cost. A total of 27 containers in 18 locations will be securely installed and serviced by the Contractor. Container sizes will be either a 1gallon container and/or a 3 gallons container both for liquids, solids and patches. (See table below). Bldg. The North American Industry Classification Systems (NAICS) is 562119 (Other Waste Collection) and the standard size is $41.5 million. The Product Service Code (PSC) is Q999 (Medical Other). No solicitation package will be issued. This notice of intent is not a request for competitive quotes; however, interested parties may identify their interest by providing a capability statement not later than December 15, 2021 at 17:00pm EST. The capability statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required services. A determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement in the future.
