- Combined Synopsis-Solicitation for Commercial Items Page 3 of 5 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-01 dated 12/6/2021. This solicitation is 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 333316, with a small business size standard of 1,000 employees. The FSC/PSC is 6515. The Network Contracting Office 9, 1639 Medical Center Pkwy, Suite 400, Murfreesboro, TN. 37129 is seeking to purchase Dermoscopy lenses. The Nashville VA Medical Center located in Nashville, Tennessee, Dermatology Service has the need for Dermoscopy lenses that are utilized by Telehealth. All interested companies shall provide BRAND NAME or EQUAL quotations for the following: Supplies/Services Dermoscopy lens: DermLite Foto II Pro: SLR Lens Polarized and non-polarized illumination Toggle between polarized and non-polarized illumination via button push or automatically Attaches to camera via Canon s EF/EF-S standard bayonet and Nikon s F-Mount connection Field of view range from 9 mm for typical to 20 mm in diameter Charger, flash cable Edge-to-edge clarity and resolution Delivery shall be provided no later than 60 days after receipt of order (ARO).
