Description
14. DESCRIPTION OF AMENDMENT/MODIFICATION (CON T): a. The purpose of this amendment is to extend time to submit quotes, change ordering period dates, and add to contractor s responsibility for services required. b. Date and time to submit quotes is changed from 28 December 2021 @ 10:00am CDT to 18 January 2022 @ 3:00pm CDT. See Block 11 of this SF30. c. B.2 Price/Cost Schedule, Page 5, ordering period dates are CHANGED FROM 15 December 2021 to 14 December 2026 TO READ 01 February 2022 to 31 January 2027 for CLINs 0001, 1001, 2001, 3001, and 4001. d. Performance Work Statement, Page 15, Item 4, Contractor Responsibilities, 4.1. Services Required is CHANGED FROM: 4.1.1. Provide portable, state of the art, digital equipment that can be easily moved into the operating room at the VHSO. Equipment must meet or exceed the following criteria: 4.1.1.1. Treats all stones anywhere in the urinary tract. 4.1.1.2. Has a large focal point to allow for better positioning. 4.1.1.3. Has a greater than 80% success rate, based on FDA clinical study data. 4.1.1.4. Is modular and can treat patients in the operating room. 4.1.1.5. Has the ability to do end urologic procedures before and after lithotripsy or laser or without moving the patient. 4.1.1.6. Will accommodate patients up to 400 pounds. TO READ: 4.1.1. The Contractor shall make available the following lasers, supplies, labor, and certified technician available to VHSO in accordance with a schedule mutually agreed upon by both the VA Medical Center and the Contractor. 4.1.1.1. Lithotripsy (Extracorporeal Shock Wave Lithotripsy (ESWL)) 4.1.1.2. Green Light Laser 4.1.1.3. Holmium Low and High Watt lasers 4.1.1.4. Cryoablation of the prostate 4.1.1.5. Disposable Flex Ureteroscopy 4.1.1.6. Thulium laser 4.1.1.7. Provide portable, state of the art, digital equipment that can be easily moved into operating rooms at the VHSO.
