Description
Displacing Sprinklers with Antifreeze Oklahoma City, OK, VA Health Care System (OKCVA) 1.0 IDENTIFICATION OF AGENCY The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Oklahoma City VA Medical Center (OKCVA) wishes to displace specific areas of the hospital sprinkler system with sprinkler antifreeze. Filling these areas with antifreeze would potentially prevent the hospital s sprinkler system from freezing as experienced from last winter. 2.0 OBJECTIVES Biohazard Area: Install a new Antifreeze Loop in the Biohazard Area. New antifreeze loop will come down next to west wall near main entrance door. New listed and approved antifreeze solution will be used. Roughly 35 gallons. A new 2-1/2 control valve will be installed and will need to be either wired (by others) into the fire alarm system or have a chain and lock installed (by others). Stairwell 2 Basement Loading Dock: Remove existing antifreeze and install/fill up antifreeze loop with new listed and approved antifreeze solution. Roughly 15 gallons. West Loading Dock: Remove existing antifreeze and install/fill up antifreeze loop with new listed and approved antifreeze solution. Roughly 15 gallons. South Central Area/ Pizza Place: Install new antifreeze piping components and roughly 20 gallons of new listed and approved antifreeze solution. Southeast Vestiblule Area: Install new antifreeze piping components and roughly 5 to 10 gallons of new listed and approved antifreeze solution. This area has an existing antifreeze system, tie into and pipe over to the (2) sprinkler heads that are in the south vestibule. 5th Floor Mechanical Room: Replace the existing 1 corroded ball valve and replace the existing antifreeze solution with new listed and approved antifreeze solution. Roughly 1 to 2 gallons. 3.0 DELIVERABLES 3.1 The contractor will provide a categorized and detailed report indicative of identified deficiencies and advisable corrective action options. 4.0 GOVERNMENT RECORDS 4.1 The Contractor and its subcontractors or consultants shall not disclose any information or data that is proprietary to the Government. All such information or data is reserved exclusively for use between the Government and the Contractor, including employees of the Contractor s firm. When deemed appropriate, the Government may authorize the release of certain information or data necessary for use by the Contractor in performance of this work, and in such cases specific written authorization must be given in advance. Such data so released shall not be further disseminated and shall not be considered released into the public domain. The Contractor must ensure that this is included in all tiers of subcontracts. 5.0 ADMINISTRATION 5.1 On behalf of the Contracting Officer, the COR is responsible for the general administration of individual task orders, review/acceptance of all deliverables, and technical direction required under said task order. The COR will serve as the Government point of contact concerning information exchange, submission review, and payment. Nothing said by the COR shall be construed to change contract requirements unless supported in writing by the Contracting Officer. 5.2 This entire project shall conform to 100% of all VA standards and specifications, OSHA, AIA Construction Guidelines, Joint Commission Environment of Care Standards, and applicable building codes. The Contractor shall be responsible for the health and safety of their own employees. The VA will inform the Contractor of any hazards such as designated permit confined spaces, electrical hazards, etc. which may affect Contractor employees. The Contractor must take precautions appropriate for the safety of their employees as well as provide equipment necessary for these actions. Contractor employees will notify the VA Representative before entering a designated permit required confined space, so precautions can be taken in advance of the work. Contractor Supervisor: A competent and experienced English-speaking Contractor Supervisor shall be provided by the Contractor whenever work is being performed. The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. This includes the requirement that the contractor have a Competent Person on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. Documentation of these requirements shall be submitted and approved prior to performing the work. The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. This includes the requirement that the contractor have a Competent Person on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. Documentation of these requirements shall be submitted and approved prior to performing the work. 5.3 VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced. 6.0 PAYMENTS 6.1 Payments shall be made upon receipt invoice and acceptance of materials and services. Final payment will be made upon receipt of final invoice and acceptance of materials and a Contractor's Release of Claims. 7.0 PERIOD OF PERFORMANCE 7.1 The period of performance is to start within 10 calendar days and be completed no later than 30 calendar days after issuance of the Notice to Proceed. **End of Statement of Work**
