Description
Sources Sought Synopsis: **This is not a solicitation** The Government is conducting Market Research, in accordance with FAR Part 10, to identify possible sources for two (2) Digital Refractors and two (2) Acuity Chart Systems to use in the Wright Patterson Air Force Base Medical Treatment Facility Ophthamology Department's clinical exam rooms for all pre and post-surgical patients. The details of the system's requirements are described within Attachment 1, Statement of Work.� This Sources Sought is solely for information and planning purposes. The intended purpose is to identify potential sources that may possess the expertise, capabilities, and experience to meet the 88th Medical Support Squadron (88 MDSS) requirement. It does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. Furthermore, 88 MDSS is not at this time seeking proposals, and will not accept unsolicited proposals. Respondents are advised that the Government will not pay for any information or administrative costs incurred in response to this Sources Sought. Please be advised that all submissions become Government property and will not be returned. All costs associated with responding to this Sources Sought will be solely at the responding party's expense. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. It is the responsibility of the interested parties to monitor this SAM.gov website for additional information pertaining to this Sources Sought.� *** All prospective contractors must be registered as ACTIVE in the System for Award Management (SAM) database to be awarded a DoD Contract.� Requirement: Please see Attachment 1, Statement of Work (SOW). The Government is in need of a vendor to provide two (2)�Digital Refractors and two (2) Acuity Chart Systems at the Wright Patterson Medical Center that fulfills all requirements within the SOW.�These are draft salient characteristics and technical requirements which are subject to be changed.� Capability Statement: All interested firms shall submit a response demonstrating their capabilities to produce the requested equipment to the primary AND secondary point of contacted listed below.� NAICS Code: 333314 - Optical Instrument and Lens Manufacturing Acceptable Means of Submission: All submissions must be submitted electronically to jessica.dunkin@us.af.mil AND tracy.ferrise@us.af.mil by 28 February 2022, 5:00PM EDT.� Any correspondence sent via email must contain the subject line, ""Digital Refractor and Acuity Chart System"". This correspondence must be contained in a single email, unless otherwise approved, and must not exceed five megabytes, included attachments. Emails with compressed files will not be accepted. Note that email filters at Wright-Patterson Air Force Base are designed to filter emails without subject lines or with suspicious subject lines or contents (i.e., .exe or .zip files). Therefore, if the specified subject line is not included, the email may not get through the email filters. Also, be advised that.zip or .exe files are not allowable attachments and may be deleted by email filters at WPAFB. If sending attachments with email, ensure only .pdf, .doc, .docx, .xls, or .xlsx documents are sent. The email filter may delete any other form of attachments� Delivery Destination: See Attachment 1, Statement of Work� Delivery Type: FOB Destination (As defined in FAR 2.101 - Definitions, the seller or consignor is responsible for the cost of shipping and risk of loss).� Inspection and Acceptance: Both inspection and acceptance will be by the Government at Destination.� Important Notice to Contractors: Response MUST also contain a complete description of items offered an any technical manuals or literature to clearly show that the items meet or exceed the requirements listed above. The Contracting Officer will review responses based on the factors listed in this Sources Sought and the information furnished by the Respondent.� The Government's evaluation of the capability statement received will factor into whether any forthcoming solicitation will be conducted as a full and open competition, a set-aside for small businesses, or any particular small business designation (e.g. SDVOSB, HUBZone, 8(a), SDB, WOSB, VOSB, etc.).�
